Care Coordinator

Job summary

NTGP has a fantastic opportunity for a Care Coordinator to join our multi-disciplinary team. The position will be part time and needs experience of working with EMIS system and excellent IT skills.

Main duties of the job

  • Proactively identify and work with people, including the frail/elderly and with long term conditions

  • Provide navigation of care and support across health and care services
  • Focus on supporting and delivery of personalised care to reflect local PCN priorities, health inequalities or at risk group patients.
  • Ability to run searches in the clinical system for meeting target specific goals and objectives
  • Ensure patients are contacted and directed to the right service based on practice call/recall policy and clinical system alerts
  • Managing digital services, including practice website

About us

We are large forward thinking practice of 15,000+ patients, providing quality healthcare and outcomes for our patients in the North Trafford area. We operate from two sites Chester Road and Seymour Grove.

The team at NTGP comprises of 2 GP Partners, 6 Salaried GPs, 2 Physician Associates, 1 Pharmacist, 1 FCP and 5 Nursing Team along with non-clinical team of 30+ staff managed by the Business Manager.

Our practices have been rated as Good by CQC and we take pride in delivering quality care to our patients and have a strong training and personal development ethos. We are part of North Trafford Primary Care Network one of the partners is the Clinical Director.

We are using EMIS Web and DOCMAN as our clinical system along with Ask My GP to provide digital platform to our patients.

Experience

Essential
  • Excellent IT Skills
  • Ability to learn quickly
  • Experience of using EMIS Web & DOCMAN
  • Ability to run searches within the clinical system
  • Proficient in using MS Office programs.
Desirable
  • Experience of working in Primary care
  • Experience of working in a GP Practice

Qualifications

Essential
  • GCSE grade A to C in English and Maths
  • Qualified to NVQ level 2 in Health and Social Care
  • Excellent IT Skills
  • Ability to learn quickly
  • Experience of using EMIS Web & DOCMAN
  • Ability to run searches within the clinical system
  • Proficient in using MS Office programs.
