Advanced Practitioner Pharmacist - Oncology and Informatics

Job summary

An exciting opportunity has arisen for an experienced clinical pharmacist to join South Tyneside and Sunderland NHS Foundation Trust as a patient focused Advanced Practitioner Pharmacist working in the Oncology service.

The role involves providing patient focused advanced clinical support to the Oncology team. You will work closely with the team along with other advanced practitioner pharmacist colleagues to ensure a high quality delivery of service for patients attending the Trust and will also have the opportunity to deliver pharmacist led Oncology clinics. You will also work with the informatics team to develop and maintain the electronic prescribing system for chemotherapy and provide support to the Oncology team with high risk medication prescribing and clinical validation. As such, the role involves advanced clinical practice in a high risk specialist area which will bring the opportunity for integrated team working and service development with the Oncology team.

Main duties of the job

You will have evidence of effective service delivery at a specialist pharmacist level, post-graduate study to diploma level, well-developed clinical pharmacy skills in an acute medical or surgical specialty with a prescribing qualification and evidence of advanced clinical skills development. The Pharmacy Service operates a seven day clinical service with routine support of both junior pharmacists and integrated medicines management technicians.

Job responsibilities

To lead and co-ordinate the professional clinical and pharmaceutical input into the safe and cost effective management and development of the utilisation of medication in conjunction with Trust emergency care and re-admissions service and lead consultants ensuring their safe and secure use and minimising the risk of inappropriate use.

To provide expert advice in the designated area to all directorate undertaking risk management, financial management and clinical governance activities and undertake prescribing to deliver medicines optimization and safe and effective discharge

To monitor performance of pharmacists working in this area, to undertake clinical supervision and internal and external teaching for all staff

To co-ordinate clinical service delivery within the safe and sustainable care project focused on front of house and ambulatory care pathways.

To link with antibiotic lead pharmacist to deliver effective antibiotic utilisation and initiation to reduce the incidence of Hospital Acquired Infections.

To work at an advanced level in the designated areas of practice.

Qualifications

Essential
  • Masters degree in Pharmacy
  • Postgraduate Study to Diploma level or equivalent experience.
  • MRPharmS
  • Evidence of CPD in line with requirements of GPhC
  • Can demonstrate management of resources, delivery of objectives in line with national priorities, participation in policy development and leadership of a team.
