Receptionist / Health Care Navigator

Job summary

An exciting opportunity has arisen for an enthusiastic motivated and hardworking Health Care Navigator to join the reception team at Wolseley Medical Centre. Applications are welcome from experienced receptionists who have already undergone training in care navigation. Applicants without directly relevant experience would need to be able to demonstrate exceptional communication skills and have the ability to learn new information quickly. Experience of SYSTMONE is particularly an advantage.

As a Health Care Navigator, you will work as part of a multidisciplinary team of clinical and non-clinical staff to coordinate and facilitate an effective communication link with our patients and their carers, to enable appropriate and timely responses to both urgent and non urgent health care needs. The successful candidate will share our values and have the energy, skills and enthusiasm to contribute to shaping how we support our patients.

The practice provides services for a diverse population, therefore it is essential that you are able to communicate sensitively and effectively with people from a variety of backgrounds and those whose first language may not be English.

Main duties of the job

Duties will include performing reception duties, booking appointments, dealing with patients both face to face and on the telephone therefore must have excellent customer service skills and a good telephone manner. You will need the ability to record accurate and sensitive information.

Job responsibilities

The following are the core responsibilities of the care navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels.

  • Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition
  • Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately
  • Process patient requests for appointments
  • Process repeat prescription requests
  • Initiate contact with and respond to requests from patients, team members and external agencies
  • Photocopy documentation as required
  • Input data into the patients healthcare records as necessary
  • Manage all queries as necessary in an efficient manner
  • Support all clinical staff with general tasks as requested
  • Action incoming emails when necessary
  • Scan patient-related documentation and attach scanned documents to patients healthcare records
  • Complete opening and closing procedures in accordance with the duty rota
  • Maintain a clean, tidy, effective working area at all times
  • Undertake all mandatory training and induction programmes

Qualifications

Essential
  • Educated to GCSE level or equivalent
  • Excellent communication skills (written and oral)
  • Strong IT skills
  • Clear, polite telephone manner
  • Competent in the use of Office and Outlook
  • Ability to work as a team member and autonomously
  • Good interpersonal skills
  • Ability to follow clinical policy and procedure
Desirable
  • Active signposting or Care Navigator training qualification
  • Customer Service qualification (NVQ) or equivalent
  • Knowledge of Systmone clinical system
  • Medical Terminology

Experience
