About us

The Sutton Primary Care Networks is the collaborative vehicle for the four Sutton PCNs and the former Sutton GP Federation working together as a single entity. We are a Community Interest Company that aims to reinvest surpluses back into general practice and primary care for the benefit of our practices and the people who form the communities we serve. Sutton has four PCNs whose boundaries are based on historical relationships and groupings of organisations. There are some similarities of resident's health and care need's unique to a particular PCN, and many more needs that are shared across all four PCNs.

Our four PCNs, represent all 21 GP surgeries around Sutton. Their collective footprints cover all of the London Borough of Sutton and its residents who are able to access and benefit from the services and improvements offered by Primary Care Networks. Primary Care Networks (PCNs) are a key part of the NHS 10 Year Plan, with all general practices being required to be in a network by June 2019, and ICBs being required to commit recurrent funding to develop and maintain them.

Quality Dashboard review and redevelopment:

Review the existing automated Quality Dashboard developed by the IT team: assess the current design, functionality and reporting outputs.

Redesign the dashboard to meet organisational quality, governance and compliance requirements, including the integration of HR, training, and Imms & Vaccs compliance.

Produce a revised and fully automated operational Quality Dashboard and reports, with the use of AI where it adds value.

Audit Dashboard and Audit Management System

  • Review current audit management arrangements and develop a standalone Audit Dashboard and Audit Management System that holds annual audit schedules for all services (currently Excel documents), enables audit scheduling and monitoring, captures audit outcomes, and records and tracks actions arising from audits.
  • Capture organisational themes, trends, learning and audit completion and non completion rates, with automated reminders for upcoming and overdue audits.
  • Replace the current reliance on multiple Excel spreadsheets and TeamNet audit recording arrangements, considering functionality from the previously developed audit system and incorporating lessons learned where appropriate.
  • Produce exportable automated reports with the use of AI.

Patient Experience Dashboard Review

  • Review the existing Patient Experience (PE) Dashboard; identify and resolve issues preventing current reporting.
  • Review PE information currently recorded on TeamNet and assess and implement opportunities to integrate TeamNet and Microsoft data into a single reporting solution.
  • Improve reporting of themes, trends and service-level feedback, and produce exportable automated reports with the use of AI.

Facilities Compliance, Policies and Risk Register Dashboard

  • Develop a Facilities Dashboard which extracts and utilises relevant TeamNet data, provides oversight of facilities compliance requirements and tracks compliance status and outstanding actions.
  • Develop a standalone Policies Dashboard which extracts policy information from TeamNet, records policy owners and review dates, and identifies policies due for review and overdue.
  • Develop a standalone Risk Register Dashboard which extracts risk information from TeamNet and records risks added or closed in the reporting month and those above the escalation criteria.
  • Produce exportable automated reports with the use of AI for each.

TeamNet System Review

  • Undertake a review of the TeamNet system covering configuration, user roles and access permissions, organisational and service structures, workflow functionality, data governance arrangements, reporting capabilities, system utilisation across departments, and opportunities to rationalise or replace TeamNet functionality where alternative solutions would be more efficient.
  • Provide a findings report, recommendations for improvement and a prioritised implementation plan.

Requirements Documentation and Handover

  • Undertake stakeholder engagement with Quality & Compliance, Digital and HR to establish reporting requirements, pain points and opportunities for improvement.
  • Produce a documented requirements specification for all dashboard developments and enhancements, together with system specifications and process documentation.
  • Review and document all current data sources including Microsoft Forms, TeamNet, SharePoint and any existing databases, ensuring data sources are clearly documented and reproducible.
  • Develop recommendations for data integration, automation and reduction of duplicate data entry.
  • Be responsible for the design, build, testing and implementation of agreed solutions, subject to SPCN approval at each stage.
  • Ensure solutions are maintainable by SPCN following completion of the engagement, and provide knowledge transfer and handover documentation.

Ongoing Improvement

  • Engage with stakeholders and encourage collaboration to ensure reporting is appropriate for stakeholder requirements.
  • Review feedback, working and supporting services to continually improve data outputs.
  • Identify areas of improvement based on data and outcomes insights to feed back to Service Leads.
  • Recommend opportunities for automation and efficiency improvements.

Governance and Compliance

Ensure all reports produced comply with data protection (GDPR), equality, diversity, and inclusion regulations.

Stay informed on NHS policies and latest NHS requirements for Primary Care.

Build only on SPCN's approved platforms and within its information governance arrangements (UK hosting, DPIA where required, Data Security and Protection Toolkit); no service, account or subscription is to be created in SPCN's name without written approval from the Head of Digital Services.
