Job summary Harlow North Primary Care Network are a group of five Practices within Harlow. We jointly deliver services that improve outcomes and provide positive patient experience for our population of 60,000+. As a Primary Care Network (PCN) our aim is to work collaboratively, enabling our team of Social Prescribers, Care coordinators, Clinical Pharmacists, Nursing Associates and GP Assistants … to work together with the Practices. We seek continuous improvement by strengthening our relationship with other key health and wellbeing providers to maximize the impact of our services on quality and outcomes of patient care.
