Practice Medicines co-ordinator
Job summary
An exciting opportunity has arisen for a motivated and organised individual to join the Medicines Management Team at Ansdell Medical Centre.
The successful candidate will work closely with GPs, Pharmacy Technicians, Medicines Management Assistants and other members of the practice team to support the safe, effective and cost-effective use of medicines across the practice. The role is pivotal in ensuring prescribing systems operate safely and efficiently whilst delivering a high standard of patient care.
The post holder will act as a key point of contact for medication-related queries from patients, carers, community pharmacies, care homes and other healthcare professionals. They will be responsible for processing prescription requests, supporting medication review and monitoring recall systems, investigating prescription queries, and ensuring actions are completed accurately and within agreed timescales.
Main duties of the job
Processing repeat and acute prescription requests in accordance with practice policies and procedures. Supporting medication review, blood test and monitoring recall systems. Managing medication and prescription queries from patients, pharmacies, care homes and healthcare professionals. Liaising with clinicians regarding prescribing issues and escalating clinical or high-risk medication queries appropriately. Contacting patients regarding medication reviews, monitoring requirements and prescribing queries. Supporting blood pressure monitoring programmes by recording and calculating submitted readings and identifying results requiring clinical review. Maintaining accurate records within the clinical system. Supporting medicines optimisation initiatives and service development projects. Working in accordance with information governance, confidentiality and patient safety requirements.

This is an exciting opportunity to become a key member within our dynamic medicines management team working within the GP practice serving 12500 patients. The GPs recognise that the quality of medicines management at the practice is vital to ensure safe, effective, and appropriate prescribing
The ideal candidate will have excellent communication, organisational and IT skills, be able to prioritise workload effectively within a busy environment, and demonstrate a commitment to delivering high-quality patient care. Previous experience within primary care, pharmacy, medicines management or another healthcare setting would be advantageous.

To ensure clinical resource and appointment allocation are used in the most effective way.
To support with information on complaints, concerns and incidents, working with the relevant Manager to resolve them in effective and timely way.
