Job summary

There is an opportunity for an INT and Digital Care Coordinator to join us at Apollo Primary Care Network (PCN) to develop the way we support our patients; improve the way our staff work and enhance our general practice (GP) services through the adoption and optimisation of current and new technologies. Apollo PCN is at early stages of Integrated Neighbourhood Journey and would welcome a candidate suitable for a transformational role, working in a diverse team and willing to develop skills to work with system partners.

Main duties of the job

The post holder will be responsible for supporting the use of GP clinical systems and digital technologies within primary care to improve the health of the local population and to build on the digital maturity of Apollo PCNand its member practices.

PCN has multiple projects and the post holder would be expected to provide support to the Clinical Director and team for delivery.

Job description

Job responsibilities

  • Monitor and oversee the PCN's EMIS system with particular focus on workflow and tasks, templates, rotas, clinical sessions and adapting them to INT needs.
  • Use EMIS to carry out searches and audits; and develop templates and protocols to support the PCN and practices contributing to development to Neighbourhood Team.
  • Run system reports and extract data as required by the PCN Senior Leadership Team for activity reporting, using EMIS and other data sources such as PHM dashboard, to support development of INT delivery plans.
  • Support Apollo PCN and its member practices by promoting and developing the use of clinical systems and digital products, digital inclusion.
  • Work across Practices to share good practice, identify information and IT needs (appointment booking), and implement effective systems and training. Supporting new IT options appraisal to inform Clinical Director
  • Develop a detailed understanding of clinical system processes and internal configuration and how they relate to the experience of users to ensure that the system is best configured and optimised to support users.
  • Support the development of reports, dashboards and analysis.
  • Support the use of digital tools such as PCIT, Accurx and the NHS App.
  • Support social media, website and waiting room screen content management, develop new innovations to support communication and delivery
  • Highlight any anomalies in information, ideas for new ways of working and details of any potential blockers to tasks.
  • Support the Transformation Lead in executing digital transformation projects across Apollo PCN.
  • Be a key member of the wider PCN Care Coordinator team to contribute to the development of strategic plans and generalist workload.
  • Act as an RA agent for the PCN if required

  • Experience of working with General Practice IT solutions
