PCN Digital and Transformation Lead
Job summary
We are looking to recruit Digital and Transformation Lead to drive a new vision and help develop and transform new and existing services across the PCN.
The role forms an integral part of the leadership team working alongside the PCN Clinical Director, Board and Senior Management Team to deliver the PCN strategy, identifying and implementing service transformation and innovation. The Digital and Transformation Lead will be responsible for managing agreed projects with a focus on developing strong relationships and partnerships across the healthcare system.
As an established single practice PCN with a patient population of 43000. We already have a high functioning leadership team overseeing all practice activity and business. This means the post holder will not have to negotiate difficult barriers between member practices to affect change across our 6 sites.
The role has an outward focus and will support the PCNs resilience by actively seeking development opportunities. This is a exciting role and would suit someone with enthusiasm and drive and a passion for improving outcomes for patients.
Main duties of the job
The role of the PCN Digital and Transformation Lead is to support increased access to care for patients across our large fully merged PCN. You will lead the adoption and/or optimisation of new technology and other initiatives to improve the care offer and enabling PCN staff to work more effectively to support the sustainability of general practice services.
You will support the Clinical Director and engage with a wide range of stakeholders, utilising population health data to transform primary care services and collaborative approaches to service development and delivery. Developing strong partnerships and actively supporting integrated neighbourhood ways of working.
You will be working on the implementation of identified projects, as agreed by the Clinical Director and Board, supporting the delivery of the NHS Long Term Plan and Network Contract Des and delivering local objectives and priorities.
The role will require good analytical skills and numeracy, knowledge of the NHS and Primary Care.

Key Responsibilities:
The PCN Digital and Transformation Lead will play a pivotal role in the development and continued evolution of Beacon Medical Group to support the delivery of high-quality primary care services. Working with the clinical and operational teams to identify opportunities to make positive transformation, and provide the inspiration, resource, and tools to execute changes in a collaborative way.
Main Responsibilities:
To support the clinical director and Board in the delivery of the PCN Contract DES.
To provide leadership and support for an agreed portfolio of improvement and service transformation projects
Support the PCN Clinical Director and Board with the writing of business and development plans
Act as a point of expertise, translate national and local policy into meaningful changes, and will support the PCN to achieve improved outcomes for the local population.
Support the Clinical Director and Board, to ensure all relevant national strategies, policies, targets and frameworks are fully implemented
Lead on transformation and innovation of services and digital projects across the PCN
Improve adoption and optimisation of new technology to enhance patient access and experience and increase PCN productivity
Attend cross organisation meetings as required e.g. Collaborative Board, Local Care Partnership
Deliver projects to support the PCN and improvements to health outcomes for the patient population
Write and submit project reports for the Board as required, showing progress against milestones, status, issues and risks
Oversee the roll out and delivery of new services, working alongside the Head of Operations and Operations Managers
Be a champion for best practices, learning from experience and from others, supporting the spread of innovation within the PCN
Develop programme and project management tools, reports and templates, that are transferable between projects, and can be used for the whole project lifetime, from design to delivery.
Manage delegated budget in line with organisational procedures
To be responsible for your own continuing self-development, undertaking training as appropriate.
To undertake other duties appropriate to the grading of the post as required.

Transformation:
Develop plans for projects identified by the PCN board, utilising the support of the PCN Business Analyst and Operations Managers and support the implementation of the projects
Work with the PCN Business Analyst and Operations Manager to ensure the PCN has a full suite of data that can be interpreted and utilised for improvement, and driving forward the PCNs strategy to support population health management
Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients
Engage with the ICB Primary Care and Digital teams to ensure local transformation plans and activity are aligned to the ICB priorities e.g. improving access, PCN Development.
Provide feedback to the management team on progress of current initiatives, capacity for change, and issues so that plans can be adapted in response
Support a programme of audits and surveys that support, and evidence measures of improvement
Understanding the PCN finances, in liaison with Director of Finance and monitoring PCN Des and other PCN income streams
Undertake full evaluation cycle of new services and service improvement
Digital:
Have management oversight of the upkeep of our website to ensure key messages are shared and patient access and navigation of the website is well planned (supported by our Patient Liaison and Communications Lead).
Improve adoption and optimisation of new technology to enhance patient access and experience and increase PCN productivity
Review and improve the PCNs digital maturity by ensuring we are using safe up to date systems that will improve our productivity and access for patient care
Support increased access to care for patients, by supporting the adoption and of new technology and other initiatives to improve the care offer and enabling practice staff to work more effectively to support the sustainability of general practice services
Training:
Work collaboratively as a key member of the PCN team, help develop and promote a positive working culture encouraging staff participation and involvement in developing their own contribution towards the success of the organisation
Offer support and training if required to leads, managers and clinicians to help support their workload
Communication:
Build relationships and facilitate collaboration between PCNs, practices and the wider system to support delivery of care to patients, maintaining credibility with key contacts and system colleagues
Represent the network at meetings, in the community as required.
Translate and present information into clear and appropriate formats, such as written documents and presentations to ensure understanding across a range of audiences
Be responsible for sharing information and best practice and persuading and influencing change in services with clinicians and a range of other stakeholders.
The information which the post holder will be required to communicate will be of a highly complex, sensitive, and potentially contentious nature. The post holder will need to use persuasive and motivational skills to achieve improvements in services where there is resistance to change.
Management:
Support workforce development planning in relation to new services, service improvements and service redesign
Work with clinical leads on the management and development of clinical ARRS staff including: Workforce development including the review of population health needs and skill mix assessments to identify future workforce needs, support training framework developments in line with the PCN contract Des
