Job summary
Bulwell and Top Valley PCN (Lead Employer: Rise Park Surgery) is recruiting an enthusiastic and highly organised Extended Access & PCN Services Coordinator to support the delivery and ongoing development of services across our network.
This is an exciting opportunity for an individual with excellent administrative, organisational and communication skills to play a key role in coordinating our Extended Access Hub and supporting the effective delivery of PCN services. Working closely with GP practices, clinicians, PCN staff and external stakeholders, the successful candidate will ensure services are delivered efficiently, appointments are managed effectively, and performance data and reporting requirements are maintained to a high standard.
The role requires a proactive and flexible approach, with the ability to manage multiple priorities in a fast-paced primary care environment. Previous experience within healthcare, general practice, NHS administration or service coordination would be advantageous.

Annual leave entitlement will be 25 days plus statutory bank holidays.

Main duties of the job
The Extended Access & PCN Services Coordinator will support the delivery of high-quality services across Bulwell and Top Valley Primary Care Network. The role will be responsible for coordinating the day-to-day operation of the Enhanced Access Hub, managing clinical rotas and appointment schedules, supporting service delivery, and liaising with practices and clinicians to ensure clinics run effectively.
The post-holder will also provide administrative support to the PCN leadership team, including coordinating meetings, preparing agendas and minutes, maintaining records, producing reports, and supporting PCN projects and service improvement initiatives.
Working closely with member practices and stakeholders, the successful candidate will help monitor activity, support reporting requirements, manage competing priorities, and contribute to the efficient, patient-focused delivery of PCN services.

Job responsibilities
Extended Access Hub Coordination
- Support the smooth running of the PCN Extended Access Hub.
- Coordinate Enhanced Access clinic schedules, including evening and weekend sessions.
- Create and maintain clinical rotas within SystmOne and other agreed systems.
- Ensure rotas are updated in a timely manner and communicated effectively to staff and practices.
- Liaise with practices and clinicians regarding clinic availability, changes, cancellations and cover arrangements.
- Support adjustments to clinics in response to staff absence, leave, or service changes.
- Assist with ensuring contracted Enhanced Access hours are delivered and appropriately recorded.
- Support collection and reporting of Enhanced Access activity and utilisation data to the ICB.
PCN Administration Support
- Provide comprehensive administrative support to the PCN Manager, Clinical Director and wider PCN team.
- Coordinate meetings, including booking venues/virtual links, circulating invitations, board papers and managing attendance.
- Prepare meeting agendas in collaboration with PCN leads.
- Attend meetings and produce accurate, timely minutes and action logs.
- Monitor progress of actions arising from meetings and follow up where required with PCN leads.
- Maintain organised electronic filing systems and records in line with information governance requirements.
- Support preparation of reports, spreadsheets, presentations and other PCN documentation.
- Assist in collating data and information from practices.
SystmOne Rota Management
- Maintain and update PCN rotas within SystmOne.
- Support clinic template setup and amendments as required.
- Ensure clinical sessions are accurately reflected within relevant systems.
- Produce activity and utilisation reports from SystmOne and other reporting tools.
- Assist practices and PCN staff with basic system queries where appropriate.
- Escalate system issues or concerns to the PCN Manager.
- Support coordination of last-minute clinic changes (e.g. cancellations/rescheduling)
Reporting and Data Support
- Support the PCN Business and Data Lead with data collection and reporting requirements.
- Assist in gathering information from member practices and PCN teams.
- Support monitoring of service activity against PCN contractual requirements (DES).
- Assist with maintaining accurate records and audit trails.
Communication and Partnership Working
- Act as a key point of contact for staff and practices involved in Extended Access clinics.
- Maintain effective communication with practices, clinicians, administrative teams and external organisations.
- Respond to queries in a professional and timely manner.
- Escalate risks, incidents, or operational challenges to senior staff.
- Support personlised Care Teams with project workstreams and attend community events to promote services and support delivery.
- Work collaboratively with colleagues across the PCN to support service delivery and improvement.
- Promote a positive and professional image of the PCN at all times.
General Responsibilities

Person Specification
Experience
- Experience of working in a health, social care, or similar setting.
- Experience of working in Primary Care or a PCN environment.
- Experience with Rota Management Systems (e.g., Lantum)
- Experience with clinical systems (SystmOne/Emis)
- Experience of service coordination, project support, or population health work.
Knowledge and Skills
- Strong organisational and administrative skills, including rota/diary
- management.
- Excellent communication skills; both written and verbal.
- Ability to work independently and as part of a multi-disciplinary team.
- Competent IT skills, including the use of Microsoft Office and willingness to learn clinical systems (SystmOne/EMIS)
- Understanding of the PCN DES and Enhanced Access requirements.
Qualifications
- At least 5 passes at GCSE level
