Main duties of the job

Providing patient care in their own homes and in hospitals.

To work within a delegated caseload with the focus of facilitating behavioral change to promote and optimize self-care.

To communicate information to patients and carers regarding their health needs and associated treatment plans.

To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members.

To contribute to and support the maintaining of service standards and performance through audit and bench-marking, and to encourage colleagues and team members to do also.

To participate, as an individual and as team member, in continuous learning and reflective practice to maintain own knowledge and skills, share good practice with and provide instruction to colleagues to continually improve services.

To continue to promote patient centered care and utilize the shared decision making tools.

To actively contribute to protecting people at risk by assessing and monitoring the care environment and individuals and by following and acting on and within Safeguarding policies and procedures.

Job responsibilities

The overall aim of the service is to work with individuals to increase their knowledge, skills and confidence to manage their long-term conditions and care needs, This role initially will pay particular emphasis to diabetes, however all long term conditions will be supported.

This is a clinical role and will involve care delivery with a behavioral change focus.

You will work closely with individuals to empower independence with their health and social care needs and use the Goal setting approach to help achieve this.

The post holder:-

  • To work as part of a multidisciplinary team supporting the care of patients within the community.
  • To undertake the administration of medications that are prescribed for patients on a regular basis.
  • To promote self-care and education to patients.
  • Be able to create a professional culture which is open to change, new ideas, concepts and innovation whilst reflecting organisational strategy and values.

Clinical duties

  • The post holder will work within an agreed framework to plan, implement, and evaluate clinical and/or therapeutic activities and patient responses to activity.
  • Support and assist other staff members to deliver care as delegated and directed.
  • Demonstrate highly developed physical skills obtained through practice and training e.g., accuracy, dexterity and coordination to complete tests and assessments within agreed pathways of care and manipulate equipment.
  • The post holder will be able to make judgement involving facts or situation which may require analysis e.g., alter programmes of activities or making clinical decisions related to specific pathway of care.
  • Able to identify risk or changes in condition and is responsible for acting; advises supervising professional and accurately record all interventions.
  • Provides and receives routine or sometimes complex information using persuasion, reassurance, tact and empathy and addressing barriers to understanding in order to engage the individual in their care and gain valid consent for care, record discussions in an accurate and timely manner in the individuals patient record.
  • Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of their role.
  • Engages and actively involves the individual and, if appropriate, their family/carer, in their assessment, programme of treatment and/or care by using person-centred techniques to promote a culture of self-care, involvement and empowerment.
  • Prioritises their own workload and understands the boundaries of their practice; may plan tasks for others e.g., patients, the workload of more junior staff.
  • Be responsible and accountable for clinical administration tasks and management of resources within the clinical area e.g., stock control, basic equipment cleaning, specific clinical tasks to support the smooth running of a clinic.
  • To comply with the organisations Infection Prevention and Control requirements, including bare below the elbows dress code for staff with a direct care delivery role.

Professional Duties

  • Have demonstrated the proficiency to administer medications and undertake annual medicines management training.
  • To be aware of and adhere to Trust Policies relating to the control and administration of drugs.
  • To ensure that high standards of care are given and maintained and to act when standards are not being maintained.
  • To work within the policies, procedures and guidelines of Calderdale & Huddersfield Foundation Trust.
  • To participate in regular staff meetings.
  • To be flexible to work within areas or departments to meet the demands of changing service needs.
  • To provide mentorship and preceptorship to newly qualified staff and students, new starters and Trainee Nursing Associates encouraging an effective learning environment.
  • To provide support and supervision to Health care support workers to promote their learning and development within role.
  • To receive delegated duties from the supervising RN and senior colleagues, accepting those that fall within their individual competency level adhering to the NMC Delegation and Accountability tool (NMC Code 2018).
  • To delegate appropriately to other team members, including Health Care assistants, family members and other lay carers adhering to the NMC Delegation and Accountability tool (NMC Code 2018).

  • To participate in regular reviews of medication errors and assist and decide action plans within the reviews.

  • To communicate with the nurse in charge and collect data in order to participate in the smooth running and organisation of the department.
  • To support junior and senior colleagues in their roles and be resilient to manage change.
  • To support the department nurse in charge in risk assessment and minimization. Ensuring incidents and near misses are reported.
  • To manage verbal complaints, inform nurse in charge and refer on when unable to resolve.
  • To participate in the investigation of incidents/complaints as required.
  • To establish and maintain effective communication, both verbal and written and relationships with multi-disciplinary team.
  • To recognize, prevent where possible, and manage areas of conflict, reporting to nurse in charge if unable to resolve.

Person Specification

QUALIFICATIONS / TRAINING

Essential
  • GCSE Maths and English Grade 9-2 (A* - E) or equivalent
  • Care qualification to NVQ 3 or equivalent experience
  • Must be willing to participate in any relevant training/qualification identified to develop skills required to carry out duties e.g., Foundation degree, Higher National Diploma
Desirable
  • Have completed training or study in areas relevant to the role applied for or be willing to undertake
  • Evidence of previous learning related to clinical area to diploma level
  • Evidence of continuing professional development.

KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential
  • Experience of working in health or social care settings without direct supervision
  • Experience of relevant clinical area e.g., older people, pharmacy, Community.
  • Experience in participating in clinical supervision.
  • Experience of organising and prioritising own workload.
  • Relevant experience of as part of a team.
  • IT awareness and ability to use SystmOne.
  • Relevant experience of working with patients and families from a range of cultures and backgrounds.
