Care Navigator

Job summary

We are looking for a part time Care Navigator to join our small, friendly Reception Team.

Experience in the NHS or a public facing role is desirable.

A minimum of Maths and English GCSE or equivalent are required.

Strong IT and communication skills are essential

Main duties of the job

The role will include dealing with telephone calls and queries, signposting patients to the most appropriate service and/or booking appointments as appropriate.

Administrative tasks will include handling prescription requests, dealing with incoming and outgoing mail, patient registrations and other basic admin tasks as they arise.

Job responsibilities

You will be required to man our reception desk, checking patients in and directing them to their appointments.

You will answer calls to the practice, assess patient needs and direct them to the most appropriate NHS service or booking appointments with clinicians as required and dealing with any queries or requests received.

You will process incoming prescription requests, clinical documents and mail as necessary.

Other administrative tasks will include completing patient registrations, actioning tasks and requests from clnicians and liaising with patients and other team members/ NHS services as required.

Person Specification

Qualifications

Essential
  • GCSE grade C or above in English and Maths ( or equivalent).
  • An Understanding of Confidentiality and data protection
Desirable
  • Experience in an NHS setting or in a public facing role.
  • Experience in inputting data and maintaining records ideal
  • Knowledge of clinical systems, in particular Emis web
