Patient Services Coordinator (PTCDA)
Job summary
Charnwood GP Network delivers the Pre-Transfer Clinical Discussion and Assessment Service (PTCDA) service to patients across the whole of Leicester, Leicestershire, and Rutland (LLR). The PTCDA model provides community-based management of patients with significant frailty, complex co-morbidities, and palliative and end-of-life care needs.
This exciting opportunity will involve working closely with the Managing Director, Clinical Lead for our PTCDA Service, Visiting GPs and the Senior management team to co-ordinate the visits and care for this group of patients.
The Patient Services Coordinator will play a crucial role in supporting the operational delivery of this service ensuring the seamless coordination through liaison with secondary care, visiting GPs, care homes, patients, and their families.
Main duties of the job
The post holder will be highly organised and proactive and can work flexibly in a fast-paced healthcare environment. The post holder must be able to manage competing priorities, provide excellent administrative support and contribute to service development.
The Patient Services Coordinator is responsible for:
Receiving and acknowledging referrals received from secondary care.
Liaising with care homes, visiting GPs, patients, and their families to arrange visits.
Actioning referrals, updating patient notes and co-ordinating ongoing care following patient contact
Arranging Multi-Disciplinary Team meetings as required
Supporting effective rota management of staff working within the PTCDA service
Completing reporting and data collection templates as required
Ensuring medical equipment is maintained and in good working order.
Assisting with risk assessments and incident reporting procedures and complaints
Managing mailboxes.
Promoting a culture of continuous improvement. Building and maintaining effective relationships with healthcare providers, commissioners, and other key stakeholders. Acting as a liaison between various teams, ensuring effective communication, information sharing and collaborative working.
About us
Charnwood GP Network is a Federation of 22 General Practices covering the localities of Charnwood in the area in and around West Leicestershire. The network encompasses four Primary Care Networks (PCNs) which work together collaboratively as part of the federation.
Charnwood GP Network is the contract holder for the PTCDA service, and we are pleased to expand the team who provide this service across Leicester, Leicestershire and Rutland.
Working days will be Monday through to Saturday. This is an office-based position, but there may be occasions where working at alternative sites may be required.

The following are the core responsibilities of the Patient Services Coordinator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
The Patient Services Coordinator is responsible for:

Person Specification
Experience
- Experience of working within Primary care/Healthcare setting at an administration level
- A good level of knowledge and understanding of the primary care sector and/or the healthcare sector
- Previous experience of supporting Senior Managers
- Completed the Making Every Contact Count (MECC) training
- At least 2 years recent experience in a similar role, either paid or unpaid.
- Evidence of having achieved personal and departmental objectives within the workplace.
- Experience of working with clinical systems and administrative IT programs and software such as (SystmOne / EMIS / Aristotle) Microsoft Office Suite
- Proven experience of Rota Management

Qualifications
- GCSEs or equivalent to include Maths and English
- Experience of working within Primary care/Healthcare setting at an administration level
- Demonstrate commitment to and focus on quality, promotes high standards to consistently improve patient outcomes
- Experience of working with clinical systems and administrative IT programs and software such as (SystmOne / EMIS / Aristotle) Microsoft Office Suite
- Knowledge of Advanced ECDL/Microsoft Office Specialist (MOS) or similar
- Previous experience of supporting Senior Managers
- Completed the Making Every Contact Count (MECC) training
- At least 2 years recent experience in a similar role, either paid or unpaid.
