PCN & Care Home Clinical Pharmacist (Alpha PCN)

This role would suit an applicant who is ideally prescribing. Initially to provide maternity cover, there may be an option to make this a permanent position in the future.

To work as part of DHPL pharmacy team to improve the population/patients health outcomes by providing direct, accessible, and timely medicines expertise to practices within Dacorum.

To pro-actively transfer workload relating to medicines optimisation issues from other clinical staff to improve patient care, safety and free up scarce clinical staff to spend more time on clinical care.

To act as a specialist and experienced point of contact/support in relation to elderly medicine and care home structured medication reviews and work very closely with the Care Home Improvement nurse(s) and GP clinical lead, member GP practices, locality pharmaceutical advisers and care home staff to deliver a programme of medicines optimisation in care homes.

To work in different GP practice and care homes throughout the working week.

To build a combination of subject matter expertise and technical skills to develop a strong service delivery within care homes.

Main duties of the job

To provide specialist pharmacy services and pharmaceutical advice across Dacorum locality care homes and GP practices, and will be involved in optimising individual resident medication regimes and carrying out wider system and medicines management process reviews in selected care homes, including developing and implementing (with wider stakeholders) system wide process changes to ensure safe care of patients in care homes, and other specific members of the local population requiring such interventions. The post holder may also be required to train other colleagues in this specialist role to promote wider sustainability of the service.

Qualifications

Essential
  • Masters degree in pharmacy
  • Member of the GPhC
  • Evidence of post qualifying and continuing professional development

Desirable
  • Postgraduate Certificate or Diploma in a clinical pharmacy discipline OR evidence of extensive clinical training and experience
  • Post graduate experience in hospital and primary care pharmacy
  • Therapeutic and clinical knowledge in the pharmaceutical care of older people
  • Extensive knowledge of specialist areas acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent
  • Must have an understanding of the background to and aims of current healthcare policy, DES contract, EHCH framework, and appreciate the implications
  • Should have an appreciation of the relationship between the DoH, NHSE, PCNs, individual service providers, and commissioning organisations
  • Member of relevant professional body
  • Non-medical Independent Prescriber
  • Undertaking or completed a relevant official CPPE NHSE pharmacist training pathway.
  • Significant experience in the use of the EMIS GP IT system and Eclipse programme
  • Comprehensive experience of project principles techniques and tools such as Prince 2 and Managing Successful Projects
  • Experience of identifying and interpreting National policy
  • Experience of researching best practice (globally, private and public sector), interpreting its relevance and processes/ practices which could be implemented successfully to achieve system reform (advising on policy implementation)
  • Previously responsible for a budget, involved in budget setting and working knowledge of financial processes
  • Experience in training others, completing clinical audits and service evaluations, supporting other HCPs and trainees educational placement visits, and applying research to clinical practice.
