Data & Digital Transformation Lead
Job summary
This role will be predominantly based at Scholes Mill , Tansley near Matlock , but with occasional remote or home working .
The Data and Transformation Lead will be a key driver in delivering the Primary Care Networks vision for integrated, sustainable, and patient-centered services in line with the NHS 10 Year Plan .
This is a hybrid role combining- Strategic transformation leadership shaping and delivering projects that move the PCN towards a neighbourhood-based, preventative, and digitally enabled model of care. - Data insight and intelligence providing accurate, timely, and meaningful data to inform decision-making, monitor progress, and evaluate outcomes. - Operational resilience ensuring smooth day-to-day functioning of the PCN and acting as cover for the Operations Manager when needed. The role will work closely with practice managers, clinicians, digital leads, and partner organisations to ensure that innovations and improvements are embedded and sustained.
Facilitate transformation workshops, collaborative planning sessions, and training for PCN teams. - Build relationships with local authority, voluntary sector, and patient groups to strengthen partnership working.
Main duties of the job
Key ResponsibilitiesTransformation and Change Leadership
Lead the design, delivery, and evaluation of PCN transformation programmes aligned to NHS Long Term Plan priorities and local population needs.Actively promote neighbourhood workingintegrating general practice, community services, voluntary sector, and social care to deliver joined-up services close to where people live.Identify, pilot, and implement new digital tools and technologies that enhance access, efficiency, and patient empowerment.
Data Analysis and Insight
Design and maintain PCN-wide data dashboards that bring together clinical, operational, and population health information.Analyse population health data to identify high-need cohorts, unwarranted variation, and opportunities for targeted interventions.Produce evidence-based reports to support funding bids, business cases, and commissioning discussions.Monitor and evaluate the impact of new services and pilots, recommending adjustments based on evidence.Uphold all data protection, NHS information governance, and confidentiality standards.
Operational ManagementSupport operational planning and performance monitoring for the PCN, ensuring alignment with contractual requirements and service standards.Step in to cover the PCN Operations Manager during absence.
Stakeholder Engagement and Communication
Act as the data and transformation point of contact for practices, Place-based partners, and Integrated Care System (ICS) colleagues.

Derbyshire Dales Primary Care Network ( DDPCN) is a established PCN which has an excellent reputation for delivering services and supporting its stakeholder GP practices and other health care providers across the entire Derbyshire Dales area .
The offices are based at Scholes Mill Tansley which is a beautiful venue just outside Matlock in the heart of the picturesque Derbyshire Dales .
The offices can be easily accessed via a good road infrastructure .
The successful applicant will be joining a cohesive group that has a 'one team ethos ' approach as a core value and is fully supportive of all staff with the proactive 'back office' function that is onmi competent accross all the areas of teh PCN business activities .

Person Specification
Experience
- Proven experience in data analysis, service transformation, or project management in a health or care environment.
- - Strong understanding of the NHS Long Term Plan, Fit for the Future agenda, and the role of Primary Care Networks.
- - Competence in data analytics and presentation tools e.g. Excel, Power BI, Tableau.
- - Strong influencing and stakeholder engagement skills.
- - Experience of operational oversight or service delivery management.
- - Ability to work under pressure and adapt to rapidly changing priorities.
- Experience in primary care, community services, or integrated care systems.
- - Knowledge of GP clinical systems and NHS data flows.
- - Formal project management qualification (PRINCE2, Agile, or equivalent).
- - Experience in leading digital health initiatives.
