Job summary

Devonport Health Centre is recruiting a Nurse Practitioner to join our team. We have been located on the Cumberland Campus for the last 11 years providing care to the patients of Devonport.

The successful candidate will be qualified to ANP standard and ideally be a Non Medical Prescriber or willing to undertake the course in the very near future.

We have staff allocated parking on the Campus.

Main duties of the job

Clinical knowledge and skills including:

  1. Chaperone procedure
  2. Requesting and processing pathology tests
  3. Advising patients accordingly
  4. Diabetes
  5. Hypertension
  6. Asthma
  7. Spirometry
  8. CHD
  9. Womens health (cervical cytology, contraception, etc.)
  10. Ability to work within own scope of practice and understanding when to refer to GPs
  11. Good clinical system IT knowledge and the ability to record accurate clinical notes
  12. Understanding of safeguarding adults and children
  13. Effective time management (planning and organising)
  14. Demonstrate personal accountability, emotional resilience and work well under pressure
  15. Ability to follow legal, ethical and professional policies/ procedures and codes of conduct
  16. Knowledge of IT systems including the ability to use word processing skills, emails and the internet
  17. Ability to identify risk and assess/manage risk when working with individuals
  18. Ability to work as a team member and autonomously
  19. Good interpersonal and organisational skills
  20. Sensitive and empathetic in distressing situations
  21. Knowledge of and ability to work to key policies and procedures

About us

Our team work well together to ensure a high standard of clinical care is provided. We comprise of 5 GPs, ACP, Clinical Pharmacist, Mental Health Practitioner, 6 members of the Nursing Team and 10 Admin. We have structured processes which promote supportive working across all members of the practice.

Job responsibilities

The following are the core responsibilities of the ANP in delivering health services.

  1. Develop, implement and embed health promotion and wellbeing programmes
  2. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  3. Implement and evaluate individual specialised treatment plans for chronic disease patients
  4. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
  5. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  6. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
  7. Request and collect pathological specimens. Upon receipt, to process and interpret pathology and other test results as required
  8. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  9. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively
  10. Develop, implement and embed well woman and well man clinics
  11. Chaperone patients where necessary
  12. Prioritise health issues and intervene appropriately
  13. Support the team in dealing with clinical emergencies
  14. Recognise, assess and refer patients presenting with mental health needs
  15. Support in the provision of vaccination programmes for both adults and children
  16. Be an extended and supplementary prescriber, adhering to extant guidance
  17. Contribute to practice targets (QOF, etc.), complying with local and regional guidance
  18. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
  19. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
  20. Deliver opportunistic health promotion where appropriate
  21. Provide holistic support in the clinical governance spectrum assuming a senior role in its compliance
  22. Support the IPC lead for the practice, ensuring compliance
  23. Monitor and ensure the safe storage, rotation and disposal of medications
  24. Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff
  25. Participate in local initiatives to enhance service delivery and patient care
  26. Support and participate in shared learning within the organisation
  27. Develop an area of specialist interest, taking the lead within the organisation
  28. Continually review clinical practices, responding to national policies and initiatives where appropriate
  29. Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required


Person Specification

Qualifications

Essential
  • Registered Nurse with Nursing and Midwifery Council (NMC)
  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice (ALNP) or, a degree for Advanced Practice Qualification up to December 2020
  • Has evidence of working at an enhanced level, as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Confirmation of registration with the NMC
  • Minor illness qualification
  • Understanding of general practice and the wider NHS
  • Experience of practice within the four pillars
  • Experience of leading in chronic disease management
  • Experience of prescribing and undertaking medication reviews
  • Experience of infection prevention and control measures
  • Experience of quality initiatives, i.e., benchmarking
  • Understanding of the importance of evidence-based practice
Desirable
  • Qualified Independent Nurse Prescriber on the NMC register
