Digital Transformation Lead - Sale Central PCN / Firsway Health Centre

Job summary

We have a fantastic new job opportunity for a full-time post with the hours equally split between Sale Central Primary Care Network (PCN) and Firsway Health Centre.

The Digital Transformation Lead supports the planning and delivery of digital improvement across the PCN. The postholder will work closely and collaboratively with the PCN's 5 GP practices, multidisciplinary teams, system partners and external stakeholders to: develop a digital strategy for the PCN; improve the use of digital systems, tools and data; optimise workflows to enhance patient care and improve patient access. They will also help the practices to enhance operational efficiency, staff experience and staff confidence in using technology.

The postholder will be responsible for implementing digital solutions aligned to NHS priorities, including access improvement, interoperability, population health management and digital inclusion. The role will provide operational leadership for digital projects, support change management, improve digital capability across practices and ensure the effective adoption of new technologies.

The post is initially a fixed-term contract for 12 months with the possibility of an extension.

    Key responsibilities

    Prepare and maintain a Digital Strategy for the PCN

    • Prepare and maintain a Digital Strategy for the PCN, informed by:
      • A full stocktake of the current digital systems and infrastructure across PCN practices (baseline position);
      • Review and assessment of future digital needs and priorities, developed in consultation with stakeholders from each practice; and
      • Identification of opportunities to transform primary care services using digital technology, including artificial intelligence, as an enabler.
    • Translate the Digital Strategy into a clear, prioritised delivery roadmap and oversee its implementation across the PCN

    Lead Digital Improvement & Support at PCN and Practice Level

    • Identify and deliver opportunities to improve workflows, productivity and patient access through digital solutions
    • Support the adoption, optimisation and effective use of digital tools, including online consultation systems, cloud telephony, digital triage and appointment navigation
    • Promote equitable access to digital healthcare services and work with practices to identify and address digital exclusion.
    • Provide handson support to staff, including training, troubleshooting and embedding new digital workflows
    • Promote consistent and standardised digital processes
    • Develop, maintain and update user guides, SOPs and training materials.
    • Support delivery of national and local NHS digital priorities through coordinated digital improvement activity.

    Project Coordination

    • Coordinate digital transformation projects across the PCN and Firsway Health Centre, with clearly defined timelines, actions and outcomes
    • Develop and maintain project plans, risk logs and progress reporting
    • Work with suppliers, ICB digital teams and practice managers to support implementation
    • Support testing, onboarding and rollout of new digital systems and solutions.

    Data Analytics & Insight

    • Collect, analyse and present data to support operational and strategic decision making
    • Create, run and report from EMIS searches
    • Build EMIS templates and protocols
    • Develop, maintain and update dashboards to support performance monitoring
    • Monitor digital KPIs, including access, telephony performance and appointment demand
    • Support data quality improvement and contractual and DES reporting requirements

    Stakeholder Engagement

    • Build strong working relationships with practice teams, clinicians, admin staff, and external partners.
    • Act as a point of contact for digital queries and support needs.
    • Facilitate sharing of best practice and lessons learned.
    • Support collaborative working across practices and system partners

    Governance & Compliance

    • Ensure digital systems and processes comply with Information Governance, cyber security, and NHS Digital standards.
    • Maintain accurate documentation, training materials, and process guides
    • Support practices with supplier management and contract monitoring

    Skills and Attributes

    Essential

    • Experience working in primary care, the NHS or a comparable healthcare environment
    • Strong understanding of general practice workflows and digital systems
    • Strong IT skills and confidence supporting others with digital tools
    • Strong experience undertaking EMIS searching and reporting
    • Strong experience in building EMIS templates and protocols
    • Experience creating dashboards and routine performance reports
    • Ability to analyse data and produce clear, actionable insights
    • Excellent communication and interpersonal skills
    • Strong organisational skills and ability to manage multiple workstreams

    Qualifications

    • Educated to A Level or equivalent experience; IT or digital training qualification.

    • Project Management Qualification (e.g., PRINCE2 Foundation), digital skills certification

    • Interpreting national policy and implementing this through programmes of work.
