Patient Care Coordinator

Job summary

This is an exciting opportunity to join our practice as a Patient Care Coordinator, a key role at the heart of our patient journey. As the first point of contact for many of our patients, you will help create a welcoming, supportive and efficient experience from the moment they contact the practice.

Main duties of the job

You will use a combination of strong administrative skills, sound judgement and compassionate communication to help patients access the care and support they need. This will include managing patient enquiries, supporting appointment booking, processing information accurately and signposting patients to the most appropriate clinician, service or community resource.

Job responsibilities

The following are the core responsibilities of the Patient Care Coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

As part of a team

First point of contact: deliver empathetic, patient-centred care including face-to-face, telephone, and our digital triage systems

Appointment navigation: use clinical software to manage appointment bookings and optimise our clinical capacity whilst adhering to our practice triage protocols

Care coordination: liaise efficiently across internal and external teams, including clinicians and multidisciplinary teams and external agencies such as secondary care and community service providers to assist with a joined up, positive patient experience

Administrative governance: execute high-accuracy data entry and patient related administrative tasks in strict compliance with GDPR and practice Standard Operating Procedures

Prescription workflow: support the repeat prescription process accurately and safely in line with practice prescribing policies and procedures

Secondary Responsibilities

In addition to the primary responsibilities, the Patient Care Coordinator may be requested to:

Participate in audits as directed by Patient Services Manager and / or senior management team

Support the Admin team, providing cover during staff absences

Complete opening and closing procedures in accordance with the duty rota
