Job summary
The General Practice Support Unit (GPSU) exists to champion, support, and strengthen general practice across Somerset. We are seeking an experienced and strategic Data Transformation Manager to lead the development and delivery of data and analytics capabilities that support primary care transformation, neighbourhood working, and population health management.
This is an exciting opportunity for a motivated individual to work closely with GP practices, Primary Care Networks (PCNs), and wider system partners to drive data-led improvement across primary care services. The role will support the delivery of high-quality analytics, meaningful insight, and improved data quality standards to help practices enhance performance, resilience, and patient outcomes.
The successful candidate will play a key role in shaping reporting and business intelligence solutions, supporting transformation programmes, and promoting the effective use of data across the Somerset healthcare system. They will bring strong analytical and stakeholder engagement skills, alongside experience working with NHS data, reporting tools, and primary care systems.

Main duties of the job
The Data Transformation Manager will lead the development and delivery of data, analytics, and reporting capabilities across the General Practice Support Unit (GPSU), supporting primary care transformation, neighbourhood working, and population health management across Somerset.
The postholder will work closely with GP practices, Primary Care Networks (PCNs), and system partners to develop dashboards, reporting frameworks, and data solutions that provide actionable insight to improve access, resilience, quality, and operational performance. Responsibilities include supporting improvements in data quality and clinical coding, developing automated reporting processes, and helping practices maximise performance and income opportunities through effective use of NHS contractual frameworks including QOF and local enhanced services.
The role will also support the development of programme outcomes frameworks and organisational KPI reporting, ensuring accurate, timely, and meaningful performance data is available to inform decision-making across primary care services.
The successful candidate will provide leadership and expertise in data analytics, support collaborative working across organisations, and promote best practice in information governance, data management, and continuous improvement

Job responsibilities
1. Neighbourhood & Population Health Strategic Data Leadership
- Lead the analytics workstream supporting neighbourhood working and integrated models of care.
- Scope and establish data access pathways required for population health management PHM, working with system partners.
- Develop and deliver analytics that support proactive care, risk stratification, and service planning.
- Translate strategic objectives into practical data and reporting solutions for frontline services
2. Access, Resilience and Quality Programme Analytics
- Lead the development of comprehensive analytics offer for the Access, Resilience and Quality ARQ programme.
- Design and build dashboards that provide practices with clear, actionable insights into:
- Access and demand
- Workforce utilisation
- Clinical activity and outcomes
- Operational performance
- Enable practices to understand how am I doing? and identify areas requiring improvement or additional support.
- Ensure reporting aligns with national and local priorities
3. Data Collection, Reporting & Insight
- Design and implement programme-specific data collection templates across practices and PCNs.
- Develop automated reporting pipelines and dashboards using appropriate BI tools e.g. Power BI.
- Ensure data outputs are timely, accurate, and user-friendly for a range of stakeholders.
- Provide advanced analytics and insight to inform decision-making at practice, PCN, and system level.
4. Clinical Coding & Data Quality Improvement
- Lead initiatives to improve standardisation of clinical coding across general practices.
- Support practices to improve data quality, completeness, and consistency.
- Develop guidance, training, and tools to embed best practice in data recording.
- Monitor and report on data quality metrics and drive continuous improvement.
5. Income Optimisation QOF & Local Contracts
- Ensure practices are effectively recording activity to maximise income under:
- Quality and Outcomes Framework QOF
- Local enhanced services LES and contractual frameworks
- Provide practices with insight and reporting to identify missed opportunities.
- Work closely with practices to improve performance against financial and quality indicators.
6. Transformation Outcomes & System Frameworks
- Lead the development of robust transformation programme outcomes frameworks, defining measurable outcomes, success criteria, and benefits realisation.
- Develop and implement processes to track delivery against outcomes, identifying risks and areas requiring intervention.
- Produce clear reporting that demonstrates programme impact at practice, PCN, GPSU, and system level.
- Collaborate with ICB and system partners to co-design and contribute to the Integrated Care System ICS outcomes framework.
- Ensure alignment between local programme metrics and system-wide indicators.
- Facilitate data sharing and integration across organisational boundaries to support population health improvement.
- Provide expert advice on outcome measurement, evaluation methodologies, and benefits realisation.
7. GPSU Performance Management & KPIs
- Lead the development of a GPSU-wide KPI framework to measure delivery, performance, and impact of services.
- Define and monitor key delivery metrics across GPSU programmes and functions.
- Ensure KPIs align with organisational objectives, system priorities, and contractual requirements.
- Develop dashboards and reporting mechanisms to track GPSU performance and support decision-making.
- Provide regular performance reporting to senior leadership, highlighting risks, trends, and opportunities for improvement.
- Embed a culture of continuous improvement through data-driven performance management.
8. Stakeholder Engagement & Collaboration
- Build strong relationships with GP practices, PCNs, ICB teams, and system partners.
- Act as a trusted advisor on data, analytics, and reporting.
- Present complex data insights in an accessible and meaningful way.
- Support adoption of new tools, processes, and ways of working.
9. Team Leadership & Management
- Line manages and support at least one Data Lead Officer / Analyst.
- Provide leadership, professional development, and mentorship across the Data and Digital Team.
- Allocate resources effectively to meet programme priorities.
- Foster a culture of collaboration, innovation, and continuous improvement.
- Contribute knowledge to other team projects and support delivery where skills and experience are relevant.
10. Governance, Compliance & Best Practice
- Ensure all data handling complies with:
- NHS information governance requirements
- GDPR and data protection legislation
- Promote robust data governance, security, and ethical use of data.

Person Specification
Analytical and Judgement Skills
- Strong experience in healthcare data analytics, ideally within NHS primary care.
- Advanced analytical skills with the ability to interpret complex datasets and generate actionable insight.
- Strong technical skills in data manipulation, querying e.g. SQL, and visualisation tools e.g. Power BI.
- Understanding of population health management and healthcare datasets e.g. GP systems, SUS, demographics.
- Ability to design data collection templates, reporting frameworks, and dashboards.
