We are delighted to offer an exciting opportunity for a motivated and compassionate Community Support Worker to join our friendly and dedicated Tewkesbury Later Life Team . This full-time Band 4 role is ideal for someone who is passionate about supporting older adults living with severe mental illness or a diagnosis of complex dementia toregain stability, independence, and an improved quality of life.

As a key member of our multidisciplinary team, you will work closely with service users, placing their needs and aspirations at the heart of every intervention. You'll provide practical and emotional support, helping individuals develop confidence, learn new skills, and engage with their communities in meaningful ways.

You will collaborate with a wide range of internal colleagues--such as nurses, occupational therapists, psychologists, and peer workers--as well as external health and social care partners, ensuring a coordinated and person-centred approach to recovery. No two days are the same, and you will play an essential role in delivering high-quality community-based care.

Main duties of the job

  • Provide care coordination for an allocated caseload of service users, ensuring their needs are assessed, reviewed, and met through personalised care plans.

  • Deliver goal-focused interventions to support individuals in developing skills, confidence, and independence.

  • Facilitate and contribute to group work and therapeutic clinics, supporting service users to engage in structured activities that promote wellbeing.

  • Work collaboratively with internal team members and external partners to ensure high-quality, person-centred care.

  • Maintain accurate, timely documentation in line with professional and organisational standards.

  • Participate in multidisciplinary discussions, contributing to risk assessments, care planning, and holistic treatment approaches.

The Band 4 Community Support Worker plays a vital role within the Later Life North Team, supporting adults living with complex dementia, behavioural and psychological symptoms of dementia or functional mental health problems i.e. complex non psychotic and psychotic new onset symptoms Working as part of a supportive and multidisciplinary team, the post-holder delivers person-centred, community-based interventions, ensuring service users are actively involved in their own care and treatment.

You will work closely with internal colleagues and external partner agencies, helping to coordinate care, promote independence, and encourage engagement with a range of therapeutic and practical support services. Main Responsibilities
  • Care Coordination: Hold a small caseload of service users, completing assessments, developing person-centred care plans, and regularly reviewing progress.

  • Deliver Goal-Focused Interventions: Provide structured, recovery-oriented support that helps individuals build confidence, develop coping strategies, and work toward personal goals.

Person Specification

QUALIFICATIONS

Essential
  • NVQ level 3 or equivalent level of competency-based training in a relevant health or social care area PLUS theoretical and practical knowledge to diploma level equivalent, obtained through formal competency training, short courses and experience OR successful completion of approved associate practitioner training or relevant foundation degree or commitment to obtain within 2 years.
  • IT Skills - including use of Rio Clinical platform
  • Can evidence significant Community Mental Health experience

LENGTH AND / OR NATURE OF EXPERIENCE

Essential
  • Experience in mental health or social care services at Bands 3 or 4, ideally in a community setting.
  • Knowledge and understanding of care planning and risk assessment
  • Knowledge and understanding of Working with Later Life adults and those with a diagnosis if dementia
Desirable
  • Knowledge and understanding of Voluntary Care Sector and partnership working
