Job summary

Are you organised, compassionate, and motivated to make a real difference? As a Care Co-ordinator, you'll be at the heart of a dynamic Integrated Reablement Team, supporting adults to remain at home, recover independence, and avoid unnecessary hospital admissions.

This is a fast-paced, rewarding role where no two days are the same. You'll coordinate care delivery, manage caseloads, and ensure the right support is in place at the right time--helping people leave hospital sooner and stay well in their own homes. Working closely with multidisciplinary colleagues, you'll play a vital role in delivering high-quality, person-centred care across the locality.

We're looking for someone with strong organisational skills, confidence in communication, and a proactive, flexible approach. You'll need to be comfortable using digital systems, able to prioritise effectively, and committed to continuous learning (NVQ Level 3/4 or equivalent).

Main duties of the job

*Manage the reenablement caseload, ensuring work is allocated efficiently and adapting to changes in service demand.

*Create and maintain staff rosters using the electronic patient record system.

*Communicate effectively with patients, relatives, and the multidisciplinary team, using appropriate communication methods for complex or sensitive situations.

*Maintain clear, concise, and compliant documentation in line with organisational policies, including accurate patient records and consent verification.

*Provide timely information to internal and external teams, including Community Nursing, Therapy, IDS at QAH, and other agencies as required.

*Deliver clear handovers and escalate clinical risks or concerns to a registered practitioner promptly.

*Contribute to team objectives, service development, and efficient resource use, attending meetings and supporting quality improvement activities.

*Use information technology effectively to support highquality care delivery and complete administrative tasks such as preparing notes, charts, and obtaining results.

*Participate in relevant training, apply new knowledge to practice, and reflect on the use of skills and competencies to meet evolving service demands.

*Support the delegated management and delivery of assessment, reablement, and rehabilitation services, ensuring interventions are monitored and support plans updated as goals progress.

Qualifications

Essential
  • *NVQ level 3/4 or equivalent.
  • *Full Driving licence

Experience
