Complex Care Team Administrator

Job summary

We have an exciting new role as a part-time administrator supporting our established and welcoming Complex Care Team. This is a neighbourhood working project, supporting people with frailty and complex care needs in the community. Working eighteen hours a week based at Harbourside Family Practice, you will be working with a dedicated team of staff from practices within the Gordano Valley Primary Care Network as well as Sirona. You will also be liaising with external agencies, including Social Care.

Flexibility around working days and willingness to travel is essential, as you will be required to attend one or more MDT meetings a week. These will be at one of the four GP practices in Portishead, Pill and Clevedon, and on different days depending on the location.

We are looking for a team player, with experience of managing a busy workload, who is passionate about supporting clinical staff to ensure best outcomes for patients.

Main duties of the job

Administrative Support for the Complex Care Team based at Harbourside Family Practice.

Liaison with external agencies who are involved in the work of the Complex Care Team.

Arranging and attending MDT meetings at four GP practices in Clevedon, Portishead and Pill.

Minuting meetings and ensuring all agreed actions are recorded and distributed.

Maintaining patient records on excel, EMIS and Black Pear.

Information governance for MDT meetings.

Support with data analysis of CTT outcomes.

The practice is part of the Gordano Valley Primary Care Network, which is made up of four practices with a population of over 50,000 patients. The Complex Care Team works across the PCN and is part of the National Neighbourhood Health Implementation Programme. The team functions inside a 'team of teams' framework that brings together general practice staff. community nurses, mental health workers, social care providers and voluntary organisations. The focus is on proactive support for individuals managing complex health conditions with frailty risks.

Job responsibilities

Administrative Support for the Complex Care Tea m

Administration of the CCT Teams channel

General administrative and IT support for the Community Specialist Practitioner (CSP) and Community Care Co-ordinators

Support clinical leads to proactively identify patients for neighbourhood support worker caseload.

Support project planning meetings

Referral Management

Accept referrals from multiple agencies and liaise with the team to allocate workload.

Record referral progress and outcomes.

MDT Meeting Support

Attend CCT MDT meetings across the four practices.

Prepare and distribute agendas prior to meetings.

Lead on information governance for MDT meetings.

Take meeting minutes and support team in the completion of care plans.

Record agreed actions on EMIS and send to team members.

Patient Follow Up & Communication

Act as point of contact for people supported by the CCT as required (phone calls and/or messages).

Prepare and communicate discharge summaries to patients via preferred method.

Data & Reporting

Support data analysts to review outcomes of the project by providing anonymised data as needed.

Support research team by providing qualitative data in the care plan as required.

Person Specification

Qualifications

Essential
  • Educated to at least GCSE grade C and above in Maths and English or equivalent (or substantial experience in a similar role will be considered)
  • Demonstrable commitment to professional development

Experience

Essential
  • Three years administrative experience
  • Demonstrable experience of working as part of a team
Desirable
  • Experience of working in a GP Surgery or in healthcare
  • Experience of using EMIS web and Black Pear
  • Experience of organising MDT meetings

Skills and Abilities

Essential
  • Excellent IT skills including Microsoft Office
  • Excellent communication skills at all levels
  • Good organisation skills
  • Ability to work on own initiative as well as part of a team
  • Full Uk driving licence and use of a car
  • Willingness to travel to Clevedon, Pill and Portishead
  • Flexibility to attend meetings on different days of the week
