Job summary
Harrold Medical Practice is seeking a compassionate, motivated, and patient-focused Practice Nurse to join our friendly and supportive primary care team.
Working alongside our GPs, nursing team and wider multidisciplinary team, you will provide high-quality, evidence-based nursing care. The role includes the delivery of chronic disease management especially diabetes care, cervical screening, immunisations and vaccinations, wound care, health promotion, and supporting patients to manage their long-term conditions.
The successful candidate will be a registered nurse with excellent clinical skills, strong communication abilities, and a commitment to delivering outstanding patient care. Experience in general practice is essential.
- Deliver safe, effective, and evidence-based nursing care.
- Manage patients with long-term conditions in line with national and local guidelines.
- Undertake cervical screening, immunisations, vaccinations, and travel health (where trained).
- Provide wound care, dressings, and removal of sutures/clips.
- Carry out health promotion and lifestyle advice.
- Support infection prevention and control standards.
- Maintain accurate clinical records using the practice clinical system.
- Work collaboratively with the wider multidisciplinary team to provide coordinated patient care.

Main duties of the job
- Undertake chronic disease management clinics.
- Perform cervical cytology screening.
- Deliver NHS health checks and health promotion advice.
- Administer vaccinations and immunisations.
- Carry out wound care, dressings, suture and clip removal.
- Perform venepuncture, ECGs and other routine clinical procedures.
- Support patients with lifestyle advice.
- Maintain accurate, contemporaneous patient records.
- Lead and coordinate the practice diabetes service.
- Undertake annual diabetes reviews, including assessment of glycaemic control, cardiovascular risk, foot assessment, and lifestyle management.
- Initiate, review and adjust treatment plans within agreed competencies, protocols and prescribing qualifications where applicable.
- Liaise with GPs, community diabetes specialists, dietitians and secondary care services to ensure integrated patient care.
- Keep up to date with national diabetes guidelines and implement changes within the practice.
Child Immunisation Lead Responsibilities
- Ensure vaccines are administered safely and in accordance with Patient Group Directions (PGDs), national protocols or prescribing guidance.
- Monitor childhood immunisation uptake and implement strategies to improve vaccination rates.
- Provide parents and carers with evidence-based information regarding vaccines and address concerns confidently and sensitively.
- Ensure the safe storage, handling and cold chain management of vaccines.
- Maintain accurate vaccination records and support achievement of national immunisation targets.

The post holder will lead the practice's diabetes service and childhood immunisation programme, ensuring the delivery of safe, effective, evidence-based care in accordance with national guidance and practice policies.
Main Duties and Responsibilities
Clinical Responsibilities
- Provide holistic nursing care to patients of all ages.
- Assess, plan, implement and evaluate individual patient care.
- Undertake chronic disease management clinics in line with national guidance and local protocols.
- Perform cervical cytology screening.
- Undertake NHS Health Checks and health promotion activities.
- Administer adult and childhood vaccinations and immunisations.
- Undertake wound care, dressings, suture and clip removal.
- Perform venepuncture, ECGs, blood pressure monitoring, urinalysis and other routine clinical procedures.
- Provide lifestyle advice on smoking cessation, healthy eating, weight management and exercise.
- Recognise and respond appropriately to medical emergencies within the practice.
- Lead the delivery and development of the practice diabetes service.
- Undertake comprehensive annual diabetes reviews.
- Monitor glycaemic control, blood pressure, lipid management and cardiovascular risk.
- Perform diabetic foot assessments and ensure appropriate referrals.

Person Specification
Experience
- Registered Nurse with current NMC registration.
- Experience working in General Practice or Primary Care.
- Competent in chronic disease management.
- Diploma/Degree in Diabetes Care or Long-Term Conditions.
- Experience of diabetes management.
- Qualified to undertake childhood and adult immunisations.
- Cervical cytology trained.
- Competent in venepuncture and wound care.
- Excellent communication and interpersonal skills.
- IT literate with experience of electronic clinical records.
- Ability to work independently and as part of a multidisciplinary team.
- Independent Nurse Prescriber qualification.
- Experience of SystmOne.
- Experience supporting Quality and Outcomes Framework (QOF) achievement.
