As Product Lead Primary / Community Care, you will hold senior accountability for the vision, strategy, and delivery of the digital product portfolio supporting primary care, community health, and integrated care services. You will work in close partnership with GPs, community clinicians, social care colleagues, operational leaders, and suppliers to ensure digital solutions are clinically safe, user-centred, interoperable, and aligned with population health and integrated care priorities.
The role includes direct line management of the Primary Care / Community Digital Team and Primary Care Product function, with responsibility for delivery, staff development, and service resilience

Job responsibilities
The Product Lead Primary / Community Care holds senior accountability for the design, delivery, and continuous optimisation of the digital product portfolio supporting primary care, community services, and integrated care pathways across Health and Care Jersey. The post holder is responsible for ensuring that digital products supporting general practice, community nursing, therapy services, social care interfaces, and wider integrated care are clinically safe, user-centred, interoperable, and aligned with organisational strategy and population health priorities. Digital solutions in this domain directly influence access to care, continuity, equity, demand management, and staff efficiency across geographically distributed services.
The post holder will:
Provide strategic leadership of the digital product portfolio for primary and community care, ensuring alignment with integrated care strategy, population health priorities, and service transformation programmes.
Act as the recognised digital product authority for primary and community care, working in close partnership with GPs, community clinicians, operational managers, social care colleagues, other Product Leads, suppliers, and technical teams. The post holder owns the full product lifecycle for all relevant systems and ensures that digital capability supports joined-up care across primary, community, acute, and mental health services.
Own the end-to-end product lifecycle for primary and community digital solutions, from discovery and co-design through delivery, adoption, optimisation, and benefits realisation.
- Lead the development and delivery of clear product visions and multi-year roadmaps that improve access, safety, continuity of care, and staff productivity across community and general practice settings.
- Work collaboratively with GPs, nurses, AHPs, social care colleagues, and operational leaders to co-design digital products that reflect real-world workflows and patient needs.
- Ensure all digital products meet clinical safety standards (DCB0129 and DCB0160), information governance requirements, and interoperability standards, working closely with the Clinical Safety Officer.
- Direct line management responsibility for the Primary Care / Community Digital Team and the Primary Care Product function, carrying accountability for service delivery, staff capability, performance, and wellbeing. Operating with a high degree of autonomy, the post holder balances clinical safety, service resilience, and digital innovation across a complex, multi-site environment.
- Champion agile, user-centred design practices, ensuring digital tools are usable, accessible, and inclusive for patients, carers, and staff.
- Maintain active engagement across community sites and primary care practices, using insight and data to identify risks, opportunities, and priorities for digital improvement.
- Provide senior specialist input into business case development, procurement, supplier management, and benefits realisation, ensuring value for money and strategic alignment.
- Work closely with other Product Leads to ensure joined-up digital experiences across primary, community, acute, mental health, and partner services.
- Promote digital inclusion, supporting patients and staff to engage confidently with digital services and reducing inequalities in access.
- Line manage the Primary Care / Community Digital Team and Primary Care IT Support function, ensuring clear objectives, effective performance management, professional development, and staff wellbeing.
Person Specification
Experience
- Experience of working in or closely with primary care and/or community services for, at least, 5 years.
- Experience of developing and implementing digital products in healthcare.
- Track record of co-designing solutions with clinical and operational teams.
- Evidence of supporting service transformation through digital innovation
- Experience of line managing staff or leading multidisciplinary teams, including setting objectives, monitoring performance, and supporting
- development.
Qualifications
- Registration with appropriate body (HCPC, NMC, GMC)
- Degree or equivalent experience in digital health, informatics, product management, or healthcare discipline.
- Registered clinical qualification.
