Newcomen Primary Care Network (PCN) is seeking a Digital & Business Transformation lead to drive meaningful change across our six member practices, serving over 46,000 patients.
This is a pivotal leadership role within the PCN. You will work alongside clinical and operational colleagues to develop and deliver a clear transformation vision improving digital maturity, enhancing patient access, strengthening data quality, and embedding innovative, efficient models of care.
We are looking for someone who combines strategic thinking with hands-on delivery someone who can inspire teams, use data intelligently, and turn improvement ideas into measurable impact for patients and staff.
The Digital & Business Transformation Lead will
Improve adoption of new technologies to enhance patient access and productivity
Increase the digital maturity of the PCN and its member practices
Strengthen data quality to better understand demand, capacity, patient experience and staff wellbeing
Support population health management approaches
Coordinate PCN implementation of national and local initiatives
Ensure alignment of digital and operational transformation with ICS and national strategy
You will work closely with practice and PCN teams to understand current service models, identify challenges, prioritise improvements and deliver sustainable change.
Main duties of the job
Leadership & Management
Lead implementation of the Network Contract DES, ensuring compliance with national and local requirements
Work closely with the Clinical Director, Finance Lead and operational colleagues on strategic and operational planning
Provide line management and development support to PCN ARRS staff
Oversee induction, workforce development and digital capability building
Monitor and review PCN sub-contracts
Work closely with practices to develop an action plan in preparation for CQC/Quality visits.
Promote integrated working across practices to reduce duplication and variation
Business Transformation
Develop and deliver a comprehensive PCN transformation plan
Prioritise improvements that enhance patient access and staff experience
Develop dashboards and performance reporting across DES, IIF, QOF, Enhanced Access and other KPIs
Work with ICS partners to ensure alignment with system-wide strategies
Digital Transformation
Develop in-depth understanding of PCN digital infrastructure and business intelligence tools
Procure and manage digital tools aligned to ICS digital workstreams
Bring together and interpret data from multiple sources
Support staff training and effective adoption of digital systems
Remain up to date with national and local PCN guidance
Finance & Governance
Working alongside the PCN Finance Lead to
Maximise income from enhanced services
Support ARRS claims and submissions
Manage project budgets effectively

The Role
The post holder will work with clinical and operational teams within Newcomen Primary Care Network to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, resource, and tools to deliver changes in a collaborative way.
The postholder will deliver a combination of the following responsibilities
-Improve adoption of new technology to enhance patient access and experience and increase PCN productivity.
-Improve PCN digital maturity.
-Improve data quality to understand patient experience of access, operation efficient and staff experience at work.
-Support population health management.
-Co-ordinate PCN support in the adoption of national and local initiatives, including integrated working at neighbourhood and place level to improve access to service for patients
-Ensure that digital and operation PCN transformation is aligned with ICS and national strategies.
The post holder will support this vision to be developed in alignment with ICS strategy. The post holder will work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. The post holder will develop strong relationships with PCN member practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.
The postholder will help to develop strategies related to transformation and improvement, aligning this with larger scale strategy and initiatives, but will focus their time on the planning, implementation and evaluation of practical improvements that make a difference to patients and staff in the PCN.
Role Responsibilities
The core objective of the Digital Business Transformation Lead is to support the PCN, its practices and the PCN ARRS workforce to deliver ongoing improvement to services. The role will require experience in business service improvement with expertise in transformational change and a good understanding of general practice systems and processes, including broad familiarity with general practice digital systems and how they can be used to deliver improvement. This expertise will be used to develop and implement a strategic plan to build and implement a robust digital and business infrastructure which
A, Develops and manages the implementation of the PCN strategic plan considering changing circumstances and ensure its alignment and collaboration with other local and Integrated Care System priorities.
B, Communicates the work and plans of the PCN to practices, communities, and system partners to facilitate engagement and ensure that the PCN has a very strong reputation for openness, innovation, and success.
C, Builds relationships and facilitate collaboration between practices to support the delivery of care to each others patients, for example to delivery of enhanced access or other hub type working arrangements.
D, Drives integrated working that improves operational efficiency whilst reducing duplication and variation between PCN services and practices.
E, Supports the financial management of the PCN ensuring that funding opportunities are maximised, and financial resources are used appropriately and effectively.
F, Manages and develops the PCN ARRS workforce staff skills and capabilities both with regards to patient care and leadership reflecting staff welfare and staff experience.
G, Develops service transformation that is underpinned by a quality improvement methodology, underpinned with easy-to-understand data that supports decision making that includes but is not limited to
-demand, capacity, utilisation data, staff, and patient experience,
-enhances patient access whilst protecting our workforce,
-takes into consideration the populations health needs and health inequalities within our community,
-takes into consideration the digital maturity of the PCN and the digital accessibility for our patients
-innovative.
Main Responsibilities
Leadership and Management
-Lead and co-ordinate the PCN implementation and delivery of all elements of the Network Contract DES to ensure compliance with local and national service requirements.
-Work with the PCN Clinical Director, Finance lead and Operation staff for strategic and operational planning processes to support the PCN objectives.
-Provide line management to ARRS PCN employed staff, to ensure that they are embedded in the PCN and co-ordinated across the PCN practices.
-Ensure the induction of all new members of staff and ensure they receive training and resources on digital tools that are relevant and available.
-Develop the PCN workforce.
-Monitor and review all PCN subcontracts.
Business Transformation
-Develops and manages an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering the potential impacts on the wider organisation, and determining resource requirements.
-Drives the planning and prioritisation of changes that will improve delivery of care and improve patient and staff experience in the practices within the PCN.
-Ensures all change is ultimately focussed on improving services for patients, through patient involved service development.
-Builds relationships across the PCN to develop consensus for improvements and to ensure business and clinical staff are fully engaged throughout the process of selection, implementation, and deployment of change.
-Links with the ICS and other partners to gain buy-in for transformation and to support alignment with broader strategy and improvement approaches.
-Develop a suite of PCN dashboards that provide monthly performance analysis of KPIs across a range of areas, performance of the DES and IIF, QOF, Enhance access.
Digital Transformation
-Develop and maintain an in-depth understanding of the PCN digital infrastructure and business intelligence tools.
-Procure and manage digital tools and projects for the PCN which is aligned to ICS digital workstreams.
-Work in partnership with the ICS to identify relevant data sets and to support standardisation in the use of measures for understanding change and improvement in general practice.
-Bring together data from a variety of sources and use information to develop insights into working practices, effectively tailoring content to meet the needs of the audience.
-Interpret data packs and highlight variation to support open discussion about where variation is and is not warranted.
-Support practices and staff to effectively use existing and new technology systems and products to enable improvement.
-Ensure all staff receive training on the digital tools within the PCN.
-Remain up to date with the national and local PCN guidance.
Finance
Work alongside the PCN Finance Lead to
-Maximise income from enhanced services.
-Liaise on ARRS staff and claim submissions.
-Manage budget allocations to specific projects.
Governance
-Create and maintain accurate documentation in relation to finances, standard operating procedures, contracts, contacts, and decision making, which is accessible centrally.
-Maintain a good knowledge of key policies from the Department of Health and Social Care, NHS England and other relevant government departments and agencies, and support PCN staff to understand and interpret these locally.
Communication, relationship building and presentation skills
With responsibility for developing and supporting delivery of transformational projects across the PCN, the post holder will require expertise in communications and engagement, and specifically in bringing people together and gaining support and buy-in to change.
-Work as part of the PCN leadership and management team. Identify and develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice.
-Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges.
-Provide feedback to PCN leadership and to member practices on progress of current initiatives, capacity for change, progress, and issues so that plans can be adapted in response.

Person Specification
Experience
- A minimum of 3 years relevant managerial experience.
- Experience of project management and quality Improvement.
- Good understanding of the Digital agenda within the NHS and Social Care or Care Sector.
- Experience in delivering transformation projects across multiples sites and/or departments.
- Experience of working in general practice/PCN with a good knowledge of general practice systems, processes, and procedures.
- Experience in delivering multiple programmes of IT projects through the entire project lifecycle in a health or care setting.
- Quality improvement training.
- Project management qualification, such as PRINCE or equivalent experience.
