Patient Discharge Assistant
Job summary
The Patient Discharge Assistant will be in the forefront of planning patients safe and efficient discharge back to community be it to their own home or to other community setting. They will liaise with the nursing team, therapy team and in some cases social services teams, whist keeping close links with family members or friends.
Main duties of the job
Excellent communication skills are invaluable to this role, alongside good time management to ensure there are no delays in discharge, thinking ahead and a proactive attitude will also be key in this role.
The role involves working closely alongside the nurse coordinator, ensuring all required discharge planning has been covered prior to leaving the hospital, this will include the re-starting of care packages, contacting residential homes, or informing relatives. Booking transport if needed and referring on to other support services.

- NVQ Level 3 Venepuncture
