Digital Transformation Manager
Job summary
The Digital & Transformation Manager will support the leadership of the South Peterborough PCN but also work across all Lakeside Healthcare led PCNs to support with workstreams and initiatives appropriate to their skills.
The post holder will work with clinical and operational teams to ensure the PCN contract is delivered to the required standards in South Peterborough, deliver service improvement and efficiency, identify opportunities to make tangible positive transformation and execute change in a collaborative way.
The role holder will be key to the vision of the South Peterborough PCN but also contribute to the vision for all Lakeside Healthcare led PCNs.
Lakeside Healthcare lead four Primary Care Networks. Along with three other members of this team, the post holder will support our South Peterborough PCN.

Main duties of the job
1. Ensure the PCN contract is delivered to standard.
2. Work closely with key internal and external stakeholders at practice, PCN, ICB and neighbourhood level, to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.
3. Along with the Clinical Director, be responsible for developing and embedding neighbourhood working in line with national and local strategy.
4. Implement, embed and evaluate practical improvements that make a difference to patients and staff.
5. Support project implementation and embedding, Quality Improvement (QI) processes, and coordinate and directly support the delivery of transformation activity at South Peterborough PCN across the PCNs as appropriate.
6. Optimise the use of technology in patient care and supporting processes.
7. Promote population health management and address health inequalities and championing diversity, inclusion, and sustainability of general practice.

Qualifications
- Educated to Degree qualified or significant evidence of academic study, work, or research.
- Evidence of continuing professional development
- Educated to Masters degree level or equivalent professional experience.
- Professional qualification or experience in IM&T or equivalent experience to demonstrate subject matter expertise.
- Leadership and management qualifications.
- Quality improvement/change management training.
Experience
- Leadership and management experience.
- Experience of project management.
- Experience and technical knowledge of clinical systems and teams.
- Familiarity with data-driven quality improvement processes.
- Experience of interacting with internal and external stakeholders at a variety of levels.
- Experience in workforce planning, forecasting and development.

- Line Management experience
- Significant leadership and management experience in a Primary Care Setting.
- Significant project management experience (small, medium, and large scale) through the entire project lifecycle from initiation to delivery within the NHS

- Experience of working in the field of digital transformation.
- Experience of developing training tools.
- Experience of CQC requirements and related practice preparatory activity.
