ED/UTC Patient Pathway Co-ordinator
Job summary
JOB SUMMARY
The post holder is one of the key members of the ED/UTC team; help maintain flow across the planned and emergency care pathways to achieve National Access Targets relevant to both areas. You will be mainly based at Ealing hospital but you may be required to work cross site if needed.
Operationally co-ordinate patient flow through the Urgent Treatment Centre, Emergency Department and CDU movement and utilisation of the bed stock within the Hospital; Continuing assessment of the bed situation in relation to capacity demand making both appropriate and pragmatic decisions for the placement of patients to SDEC, CDU and UTC. Maintain links with all divisions to promote efficient and effective working relationships, recommend actions in line with Full Capacity Protocol and Pre-Emptive transfer protocol. Work with key internal and external stakeholders to ensure the trust achieves the emergency care KPI's. Ensure the team achieves its internal professional standards and escalation through to the other specialties. Prepare reports and provide updates on the previous day's performance. Be involved in plans to mitigate any identified risks to delivery of the KPI's and provide data for breach validation of the 4 hours, escalate concerns around demand and capacity, recognising early indicators of pressure is a key part of this role, and action accordingly.

Key Responsibilities
To be responsible for providing administrative support to the clinical teams by keeping Cerner up to date and maintain an accurate bed state in UTC and ED, through face-to-face communication with the Nurse in charge in UTC, ED and acute floor.
To ensure every patient is tracked to an external or internal pathway and treated as quickly as possible, ensuring this is achieved within the national 4-hour target.
Maintain and up-to-date record of patient tracking on Cerner and escalate to the Nurse in charge regarding bed demand, acuity, capacity pressures and waiting time within the department.
To support the Service Manager to ensure all administrative process are delivered related to admitted and non-admitted flow and ensure all actions are carried out in line with agreed process and policies.

Person Specification
Essential
- Educated to NVQ level 4 or equivalent
- Previous NHS experience
- Extensive experience of Microsoft Office applications and database Software
- Excellent understanding of the ED/UTC KPI's
- High standards of written and verbal communication skills
Desirable
- Medical terminology qualification
- Experience of working in an Acute Setting
- Previous experience of using IT systems/ patient management systems
- Experience of monitoring waiting times data
