Bexley Primary Secondary Care Interface Clinical Lead

Job summary

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The interface between primary and secondary is one of the key channels through which patients and their care flow. It needs to be patient focussed, delivering the best possible experience of care and clinical outcomes whilst ensuring that there is efficient use of resources. However, frustrations are common with all stakeholders having to contend with multiple touch points, at times inadequate communication, failure to make use of all the expertise in the system at the right time and poor use of patients' and clinicians' time. The interface is complex and relies on processes which may not be fit for purpose, incompatible IT systems and, often, poor transfer of information.

Main duties of the job

The ICB is seeking a clinical and care professional lead to support the Bexley Place system to address the 4 specific interface domains detailed within the NHS England Delivery plan for recovering access to primary care. These include: onward referrals, complete care, call and recall and clear points of contact.

The postholder will develop and expand 'Place' or 'Trust-based' interface groups to define, prioritise and address locally identified issues with a focus on building relationships across the interface to address the problems and define effective and sustainable solutions.

Person Specification

Qualifications

Essential
  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area
Desirable
  • Certificate / qualification in programme management, service improvement or transformation

Experience

Essential
  • Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation.
Desirable
  • Previous experience of being part of peer networks and collaborative working
  • Cross organisational and/or professional boundary collaboration experience

Knowledge

Essential
  • An in-depth understanding of health and care provision and awareness of current ICS/system policies and priorities
  • Champions the principles of equality, diversity and inclusion both within and outside of the organisation. Provide effective, inclusive and compassionate leadership ensuring all voices are heard and creating and nurturing a culture where staff feel safe to speak up.
Desirable
  • Good knowledge or cross professional transformation, digital solutions and innovative redesign.
