Job summary
An exciting opportunity has arisen to join the Community Nursing Service in Halton. We are looking for an enthusiastic and motivated individual who is ready for a new challenge. The successful candidate will support the team by undertaking administrative duties, receiving referrals, and liaising with patients and the wider multidisciplinary team to help deliver high-quality care to housebound patients. Applicants must have excellent interpersonal, communication and IT skills, including experience using Word, Excel and Outlook. Main duties of the job
The post holder will provide administrative support to the Community Nursing Team, helping to ensure the smooth day-to-day running of the service. Duties will include receiving and processing referrals, maintaining accurate patient records, booking appointments, managing telephone enquiries and supporting communication between patients, carers, clinical staff and other healthcare professionals. The role will involve using NHS IT systems to update information, monitor tasks, support stock ordering and assist with general office duties. The successful candidate will be expected to work as part of a busy team, prioritise workload effectively, maintain confidentiality and provide a professional, helpful and compassionate service to patients and colleagues.
Complete venepuncture training and support delivery of the phlebotomy service.

Job responsibilities
- The post holder will undertake clinical interventions under the direction of a Team leader
- Maintain accurate documentation on the IT System/records.
- Assist in ensuring equipment/environment is clean according to the Infection Control Guidelines.
- Liaise with other clinicians / administration, reporting as appropriate
- To provide clerical support for district nursing team
Duties and Responsibilities
Clinical
- Maintain and document accurate client records using the IT System/Records.
- To treat patients with dignity and respect at all times.
- To work competently performing basic clinical skills as directed by a district nursing sister, i.e. basic dressing techniques
- Monitor stock levels and re-order required items utilising Shared Business Services
- Ensure environment and equipment is clean according to Infection Control Guidelines.
- Ensure safe storage and transfer of blood samples/other to the laboratory.
- To communicate and liaise with other team members, reporting any changes in the patients condition.
- Provide support and advice to patients, relatives and carers.
- Work collaboratively with other health professionals within the clinic.
