North Hampshire Urgent Care is seeking registered clinicians to join our Remote Monitoring Service. This is an exciting opportunity to work at the forefront of digital healthcare, supporting patients through innovative remote monitoring pathways that help identify deterioration early, improve outcomes, and reduce unnecessary hospital admissions. NHUC has been delivering a Level 3 virtual ward across North and Mid Hampshire for over 5 years and is expanding to start a Level 2 proactive service across Hampshire and Isle of Wight.
The service operates 7 days a week between 08:00 and 20:00, ensuring patients receive timely support when they need it most. We are recruiting full-time, part-time and zero-hours staff who can work flexibly across a range of shift patterns, from our Head Office location in Farnborough. This includes a mixture of weekday, weekend and bank holiday shifts, offering a range of working opportunities.

This is initially a fixed term position until 31/08/2028.

Main duties of the job
Please note, this is NOT a remote position.
As a Remote Monitoring Clinician, you will use your clinical expertise to assess, monitor and support patients remotely, providing professional oversight and clinical decision-making to ensure high-quality care is delivered across the service.
Key responsibilities include:
- Managing Level 3 patient onboarding and discharge processes.
- Responding to digital platform alerts and identifying patients requiring urgent intervention.
- Supporting and supervising Clinical Support Workers, providing clinical advice and accepting escalated cases.
- Working collaboratively with GPs, community teams and wider multidisciplinary colleagues.
- Promoting self-management and empowering patients to take an active role in their healthcare.
- Contributing to service improvement, innovation and pathway development

About us
NHUC is a not-for-profit Community Benefit Society, working in partnership with the NHS. NHUC provides services through two divisions, Same Day Care (SDC) and TalkPlus.
SDC provides services through the Integrated Urgent Care (IUC) contract which covers the in Hours Clinical Assessment Service (CAS) and Out of Hours (OOH) provision for North Hampshire, North East Hampshire and Farnham & Surrey Heath places which are part of the Hampshire and the Isle of Wight ICB and Surrey & Sussex ICB respectively. We also provide an Urgent Treatment Centre at the front door of Frimley Park Hospital whilst also holding some smaller contacts supporting Community Hospitals OOH and Flu response. Finally, SDC has a virtual care arm supporting patients at home.
TalkPlus provide NHS Talking Therapies, supporting people with common mental health problems in Northeast Hampshire and Farnham place. TalkPlus offers a range of treatments to people who are experiencing symptoms of stress, anxiety, low mood, depression, panic, phobias or OCD. TalkPlus has care pathways to provide timely treatment for perinatal and military veteran referrals.

Role Summary:
The NHUC Remote Monitoring Service provides two interlinked services across North and Mid Hampshire; a proactive level 2 remote monitoring service that consists of the long term monitoring of patients with moderate/severe frailty and identifying deterioration earlier to promote community management. The service also provides a level 3 short term intensive monitoring for acute respiratory infections and exacerbations of respiratory chronic diseases.
This is a rapidly expanding area of clinical care in line with the NHS England strategic direction with the shift of care into the community and supporting proactive, not reactive care.
The post holder will work as part of a team of clinicians providing the service, with responsibility in particular for the following:
- Level 3 onboarding and discharges
- Responding to digital platform red alerts
- Responding to escalated cases from other non-registered clinicians
- Clinical oversight for the non-registered clinicians
- Oversight of cases escalated outside of the Remote Monitoring Team
Main Duties and Responsibilities:
- This role encompasses the skills of leadership and clinical oversight of the Remote Monitoring Service, using telephone and video triage to carry out assessment and monitoring within an agreed scope of practice.
- To work collaboratively with the Clinical Head of Service Primary Care, Remote Monitoring Service Manager, Lead Nurse, and other service Clinicians and Clinical Support Workers. This may include covering each other, and in some cases working closely together and sharing a role.
- To provide clinical oversight and support of the non-registered Clinical Support Workers, accepting escalated cases, providing training and clinical advice to ensure patient safety at all times.
- To use advanced clinical skills to provide education to patients, promoting self-care, and empower them to make informed choices about their treatment. To participate in service development in conjunction with local colleagues and specialist practitioners as required
- To prioritise health problems and intervene appropriately, including initiation of effective emergency care via the most appropriate services.
- Promote health and well-being using health promotion, health education, screening, and therapeutic communication skills.
- To clearly communicate the health status of the patient using appropriate terminology and technology
- To work collaboratively with the patients own GP & multidisciplinary team responsible for their care where required
- To maintain accurate and contemporaneous records, utilising electronic systems and consider the Caldicott Principles in relation to all data handling.
- To provide professional advice to multidisciplinary colleagues in relation to specialist area of practice.
- To ensure adherence to the professional rules of conduct as laid down by the NMC (Nursing & Midwifery Council), HCPC (Health and Care Professions Council) or equivalent professional body.
- The Remote Monitoring Service will be run by North Hampshire Urgent Care working alongside other key external stakeholders. Governance Structures will be determined by North Hampshire Urgent Care. Within this structure, this role will be expected:
- To understand and comply with patient confidentiality as per Caldicott Guardianship Guidelines
- To undertake and comply with Data Protection Issues
- To undertake and comply with mandatory training as required
- To have a working knowledge of and comply with all organisational policies, procedures, and guidelines
Administrative and professional responsibilities:
- Oversight of the remote monitoring platform, to ensure patients are not missed from the appropriate level of escalation/follow up.
- Participation in team meetings to discuss patients on the Remote Monitoring service as required
- Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer systems as appropriate.
- Ensure accurate completion of all necessary documentation associated with patient health care including EMIS Web.
- Keeping up to date with service information sent by email.
Liaison:
- Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines with appropriate regard to confidentiality.

Qualifications
- Current and full membership with a professional registering body (NMC, HCPC, GMC)
- Educated to degree level
- Evidence of post-registration training relevant to long-term conditions, urgent care or remote monitoring
- Postgraduate qualification in advanced clinical practice, digital health, leadership, education, management or related subject
Experience
- Significant clinical experience working autonomously
- Experience managing initial patient contacts within a clinical setting (dealing with undifferentiated demand), with clinical decision making and risk assessment.
- Experience working within urgent care, primary care, same day care or integrated urgent care services
- Experience of leading, supervising or managing non-registered clinical workforce
- Expert in managing workload and that of others in a fast paced and unpredictable environment
- Experience working within a Remote Monitoring Service, virtual wards or telehealth services
- Experience managing service users with frailty
- Proven track record of service development, transformation or pathway development programmes
- Experience of supporting CQC inspections, regulatory compliance or quality assurance processes
