Information & Change Manager
Job summary
The Information and Change Manager will play a pivotal role in supporting the strategic and operational development of the PCN. The postholder will ensure that high-quality information and intelligence are available to support decision-making, performance monitoring, service redesign and population health management.
Working closely with practice teams, clinicians, system partners and commissioning organisations, you will lead change programmes and drive continuous improvement through effective use of data and digital technologies.
Main duties of the job
Information Management
- Develop and maintain PCN performance reporting systems and dashboards.
- Analyse, interpret and present complex healthcare data to support decision-making.
- Monitor key performance indicators and contractual requirements across the PCN.
- Support population health management initiatives through data analysis and insight.
- Ensure compliance with information governance and data protection requirements.
- Produce reports for PCN leadership, member practices and external stakeholders.
Change Management
- Lead and coordinate service improvement and transformation projects.
- Support practices and PCN teams through organisational and operational change.
- Apply recognised project and change management methodologies.
- Identify opportunities for innovation, efficiency and quality improvement.
- Facilitate stakeholder engagement and communication throughout change programmes.
- Monitor project progress, risks, benefits and outcomes.
Digital and Strategic Development
- Support the implementation and optimisation of digital systems and technologies.
- Work collaboratively with Integrated Care Board (ICB) and system partners.
- Contribute to PCN strategic planning and business development.
- Promote data-driven decision making across the organisation.

Person Specification
Qualifications
- Degree level education or equivalent experience in management, healthcare management, business intelligence or a related field
Experience
- Degree-level qualification or equivalent experience in information management, healthcare management, business intelligence or a related field.
- Experience of analysing and presenting complex data.
- Proven experience leading change or service improvement projects.
- Strong project and general management and organisational skills.
- Excellent communication and stakeholder engagement abilities.
- Advanced Microsoft Excel and data reporting skills.
- Understanding of information governance, GDPR and healthcare data management.
- Ability to work independently and manage multiple priorities.
- Experience working within the NHS, primary care, PCNs or integrated healthcare settings.
- Knowledge of NHS performance frameworks and commissioning processes.
- Experience with population health management and healthcare analytics.
- PRINCE2, Agile, Lean or change management qualification.
- Experience using Power BI or similar business intelligence tools.
