Main duties of the job

  • Key Responsibilities:

    Serve as the main point of contact for processing referrals into community services.Handle a high volume of calls from various agencies in a busy office environment.Process referrals and associated correspondence promptly and accurately.Follow up on queries and ensure patient records are updated efficiently.Utilise decision-making skills to signpost service users and professionals to appropriate service areas.Facilitate effective communication and collaboration across professional and organisational boundaries.

    Working Hours:

    The Hub operates from 8am to 5pm, Monday to Friday, with shift patterns within these times.

    This role includes occasional weekend hours to manage a small number of referrals received outside normal working hours. Weekend hours are shared within the team and occur no more than once per month.

    Requirements:

Job description

Job responsibilities

  • Manage and process incoming referrals to the Hub, address queries, take detailed messages, and respond or refer them to the relevant multi-disciplinary team member as defined in the referral management protocols.
  • Ensure service users/patients are directed to appropriate community assessment support services.
  • Effectively use communication skills to enable referrers to explain needs and required assistance, while being sensitive to their feelings and respecting their wishes whenever possible.
  • Assess referrals to determine the type and priority of need, and appropriately refer to other services and agencies, seeking advice from the line manager if necessary.
  • Ensure all referral documentation contains relevant information to assist clinical triage.
  • Input data into electronic clinical records to support the flow of information between the Hub, clinical teams, and partner agencies.
  • Keep electronic systems up to date by entering and amending patient admissions, discharges, and other details to allow audit, review information, and produce relevant reports.
  • Contribute to the provision of administration services to adult care teams, prioritising and managing the workload efficiently.
  • Manage communications with other healthcare professionals in accordance with local and Trust Policy.
  • Support clinicians by collating patient information in preparation for Multidisciplinary Team meetings.
  • Ensure confidentiality in all matters related to patients and information obtained during employment.

Please note that this post does not meet the pay level required for a Skilled worker visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Qualifications

Essential
  • Educated to GCSE standard or equivalent
  • Able to demonstrate literacy and numeracy
  • NVQ Level 4 in Business Administration or equivalent experience

Experience

Essential
  • Previous experience of working in a customer focused environment
  • Experience of working in a Multi-Disciplinary Team
  • Computer literacy and experience of database management of records and information in an office environment e.g., patient record systems. High level of experience using Microsoft Office Software
  • Previous experience of working to targets/deadlines
  • Knowledge of Practice Procedures, associated systems, administration and community service provision acquired through experience and training to NVQ 4 or equivalent
Desirable
  • Experience of working in an NHS or Social Care organisation
  • Experienced user of SystmOne
  • Knowledge of clinical computer systems
  • Experience of working in a telephone focused environment
