This role is based within one of the 11 GP Practices that form the Rushcliffe Primary Care Network (PCN) and plays an important part in supporting the delivery of the Primary Care Networks key priorities. Working as part of a collaborative team, the post holder will contribute to the delivery of the NHS England 10-Year Health Plan by supporting proactive and preventative care initiatives, with a particular focus on the ongoing management of patients with long-term conditions.
The role is varied involving patient searches and data gathering, contacting patients, coordinating appointments, supporting quality improvement projects and clinical initiatives and promoting the use of Digital Health Tools. The successful candidate will be highly organised, motivated, and IT proficient, with the ability to identify innovative ways of working that improve efficiency, streamline administrative processes, and enhance patient services.
Strong interpersonal and communication skills are essential, along with the ability to work effectively within a team while managing individual responsibilities and priorities.
Main duties of the job
Main duties include running and analysing reports using the clinical system SystmOne, ensuring patient records are accurate, complete, and up to date, handling confidential information in accordance with data protection and organisational policies.
You will be responsible for identifying eligible patient cohorts through data searches, supporting the management of long-term conditions, coordinating and booking patient appointments, monitoring waiting lists, and assisting with the delivery of a range of clinical and preventative care programmes. You will provide patients with clear, accessible information to support them in managing their health and accessing appropriate services.
The role requires close collaboration with GP practice teams, clinicians, and PCN colleagues, providing essential administrative support to ensure the effective delivery of services. You will coordinate clinics, contribute to project work and service improvement initiatives and support the implementation of new ways of working that improve efficiency and patient outcomes.
The successful candidate will be confident using a range of IT systems, able to work independently when required, and capable of planning and prioritising their own workload while seeking guidance and support appropriately. The role requires flexibility and the ability to travel across all areas of Rushcliffe as required.

PURPOSE OF THE ROLE
A care coordinator works with GP practices to help run reports, organise patient visits, deal with tasks and generally help patients to get a better service from doctors, nurses, clinicians and admin teams within the practice and other professionals.
MAIN DUTIES AND RESPONSIBILITIES
Run reports from clinical systems such as SystmOne (training will be provided if needed) and interpret and analyse data.
Help patients manage their needs by answering queries, making and managing appointments, making sure patients have understandable written or verbal information to help them make choices about their care.
Maintain patient clinical records ensuring they are accurate and up to date using read coding and data entry.
Work with teams within the GP practice and those who support GP practices, providing administrative support as required.
Support or manage clinics as required including management and monitoring of services and staff rotas.
Help patients gain access to self-management education courses and peer support/interventions that support them to take more control of their health and wellbeing
Support the coordination and delivery of muti-disciplinary meetings.
Adhere to organisations policies and procedures, guided by occupational policies and procedures in primary and secondary care.
Work without supervision, plan own workload and seek guidance as required from line manager and colleagues.
Readily promote and support access of digital health tools (NHS App) to patients, communities & colleagues to help people manage their health, care and wellbeing through improved access to services.
Maintain and update PCN and GP Practice websites, social media channels and digital patient communications, ensuring timely, accurate, and accessible information is available to patients and the wider community.
Use clinical systems for record keeping, audit and evaluation.
Always maintain confidentiality.
LEARNING & DEVELOPMENT
Undertake ongoing training as required for the role of the care co-ordinator.
Take an active role in 1-2-1s and appraisals.
