Job summary

The post-holder is a Clinical Coder.

The Clinical Coder will be responsible for the clinical coding of relevant information into patients electronic healthcare records on the clinical system, adhering to the standard SNOMED CT codes and relevant organisational policies.

In addition, the post-holder will support the multi-disciplinary team, providing administrative support as required, in line with the needs of the organisation. Duties can include, but are not limited to, supporting the administration and receptionist teams with patient registration, booking appointments, the processing of information (electronic and hard copy) in a timely manner and in accordance with current policies.

The post-holder will support staff with the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.

The post-holder will be an integral part of the general practice team.

Job hours

Monday, Thursday and Friday 0930 - 1800

Main duties of the job

a. Accurately clinically code data into the patient record on the clinical system

b. Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action

c. Inputting and extracting data as required to support practice reports, audits and to capitalise upon both patient safety and QOF income

d. Scanning patient related documentation and attaching scanned documents to patients healthcare records

e. Highlight all timely actions, changes to patient care and management to the relevant clinician

f. File and store records as required

g. Photocopy documentation as required

h. Action all incoming correspondence, emails and mail in a timely manner

i. Input data into patients healthcare records as necessary

j. Answer incoming telephone calls, transferring calls or dealing with the callers request appropriately

l. Conduct system searches as requested

m. Maintain a clean, tidy, effective working area at all times

n. Support all clinical staff with general administrative tasks as requested

o. Undertake a broad spectrum of administrative duties commensurate with the role

p. Act as a chaperone as required

q. Be aware of duties and responsibilities regarding legislation and adhere to practice policies and procedures on safeguarding adults and children

r. Support in the delivery of enhanced services and other service requirements

s. Undertake all mandatory training and induction programmes

The following are the core responsibilities of the Clinical Coder in delivering health services. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

In addition to the primary responsibilities, the Clinical Coder has the following wider responsibilities:

a. Participate in local initiatives to enhance service delivery and patient care

b. Support and participate in shared learning

c. Complete opening and closing procedures in accordance with the duty rota

d. Support reception staff, providing cover during staff absences

Person Specification

Experience

Essential
  • The successful candidate will have relevant experience that demonstrates their ability to carry out the duties of the role effectively. They should be able to work independently and collaboratively, communicate confidently with a range of people, and maintain a professional and patient-focused approach. Previous experience within a healthcare setting is desirable, although transferable skills gained in other sectors will also be considered.

Qualifications

Essential
  • Good standard of education, including GCSEs (or equivalent) in English and Maths.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively as part of a team.
  • Good organisational and time management skills.
  • Ability to prioritise workload and meet deadlines.
  • Competent IT skills, including Microsoft Office applications.
  • Understanding of confidentiality, data protection and information governance.
  • Ability to work accurately and pay attention to detail.
Desirable
  • Previous experience working in a GP practice, primary care, NHS or healthcare setting.
  • Knowledge of NHS systems and processes.
  • Experience using clinical systems such as EMIS Web and Docman.
  • Knowledge of Care Quality Commission (CQC) standards.
  • Relevant qualification related to the role.
  • Experience of service improvement or quality improvement initiatives.
  • Understanding of QOF, Enhanced Services or Primary Care Network working.
  • Previous experience supporting multidisciplinary teams.
