Overview
We are seeking a motivated Care Coordinator preferably with experience as a Healthcare Assistant to work within the Integrated Neighbourhood Team at Banbury Cross Health Centre where we provide care for complex, vulnerable and often housebound patients across the Banbury area. This is an exciting and diverse role which requires you to proactively manage a list of patients as a care coordinator, but also requires elements of a health care assistant and social prescriber/link worker role.
The Team
The Integrated Neighbourhood Team is a multi-disciplinary team formed of various roles including GPs, Care Coordinators and Social Prescribers alongside other members of Banbury Cross Health Centre.
The aim of the Integrated Neighbourhood Team is to reduce unnecessary hospital admissions by having a proactive approach to managing patients in the community, involving the wider Multidisciplinary team including District nurses, community therapy services, neighbourhood frailty and visiting services, social workers, Hospital at Home and Geriatricians.
Our team gets referrals from other health care professionals and by reviewing patients post hospital discharge who meet our criteria. We offer holistic patient centred care, and collaborate with a wide range of community and voluntary services such as Age UK, Dementia Oxfordshire, Cherwell District Council, Volunteer Transport Service etc.
Main duties of the job
The Role and Main Duties
- The care coordinator/HCA will be part of our Integrated Neighbourhood Teams MDT.
- As a team we jointly look after a list of patients who have a variety of support needs. We operate Monday to Friday 9am 5pm from Banbury Cross Health Centre.
- As well as being office-based working with the team the care coordinator will do patient reviews during home visits across Banbury. You will therefore need access to a business insured vehicle with a full driving license.
The role would require you to:
- Provide care for patients in their own homes.
- Work with the registered clinicians to complete tasks and provide care.
- Supported by the clinical team and appropriate training you will deliver care within the boundaries of your role, focusing on supporting patients to be healthy, in the monitoring of long-term condition, and health prevention and screening activities.
- Be part of the wider MDT involved in patients care and attend MDT meetings.
