Band 6 Diabetes Specialist Nurse
Job summary
The Specialist Nurse will be required:
- To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries.
- To work autonomously to deliver specialist clinical care appropriate to the needs ofthe patient group and based on principles that are research based and whichenhance the quality of patient care.
- To contribute to the facilitation of the planning, delivery and evaluation of carepathways for both outpatients and in- patients with Diabetes.
- To be responsible for the care of a case load of patients requiring specialist nursingcare within the outpatient MDT and Nurse led clinics under the supervision fromband 7/8a DSNs.
- Providing holistic & psychological support, advice and information to diabetespatients and their relatives and carers both face to face and using our diabeteshelpline service and online email.
- To support staff and patient education, have an understanding of insulin pump &CGM services. This will require excellent IT skills.
- To provide specialist support to the ward areas.
- To support the nursing team to monitor the safety and quality of care for patients.
- To ensure a positive focused service incorporating patients' feedback tomaintain and improve patient care.
- To act as an effective role model promoting excellence through practice.
Main duties of the job
CLINICAL RESPONSIBILITIES
- Maintain a professional behaviour at all times and promote a positive image ofRoyal Free London NHS Foundation Trust at all times, in line with World ClassCare values.
- Ensure that the needs of patients are placed at the centre of care delivery, thusachieving trust and national standards for clinical quality, safety and patientexperience.
- To participate in and manage caseload of inpatients with the support of InpatientClinical Nurse Specialists, Lead nurses and Consultants.
- To use advanced clinical assessment skills and judgement to assess, plan,implement and evaluate care for patients with complex physical andpsychological problems using a recognised, evidence base model of care.
- Suggest and interpret relevant tests and investigations.
- Provide appropriate and timely education and advice to patients on all treatmentoptions available.
- To act as a clinical expert role model in nursing care to other nurses withregards to Diabetes.

- Recognised teaching or mentorship qualification

- Post Registration experience of nursing patients with diabetes
