Care Co-Ordinator
Job summary
Shelley Manor & Holdenhurst Medical Centre is seeking an enthusiastic, highly motivated and organised Care Coordinator who has had previous experience of working in primary care. We are looking for someone to support our more vulnerable patients through their health care journey by being a patient advocate/supporter, as well as having excellent IT skills to be able to undertake searches and reports, manipulate data in excel and communicate with colleagues to support the management of patient registers, QOF and IIF.
Main duties of the job
Our Care Coordinators provide extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations and other primary care professionals. You will work closely with the GPs, managers and colleagues within the practice to identify and manage a cohort of identified patients, making sure that appropriate support is made available to them, inviting patients for appointments, managing clinics and ensuring that their needs are addressed. The focus is on the delivery of a comprehensive model to reflect National clinical pathways, local priorities, health inequalities and patients at risk.

Shelley Manor & Holdenhurst Road Medical Centre are looking for Care Co-Ordinator to join the team. We look after 26,000 patients across 2 sites in East Bournemouth.
We are a supportive and successful training practice, with a strong ethos of staff development. You would be joining a robust team up of 9 partners, 4 Salaried GPs/Retainers, Trainees, ANPs, Nursing team, Pharmacy team, First contact physios as well as our management and admin team.

Qualifications
- GCSE English & Maths.
- Proficient in Word and Excel.
- Experience of working in a primary care setting or secondary care or NHS.
- Experience of working in an administration role.
- Experience of data collection and using tools to measure the impact of services.
- Excellent communication skills (written and oral).
- A clear understanding of child protection policy and procedures and commitment to the safeguarding of children and vulnerable adults.
- Good IT skills.
- Clear, polite telephone manner.
- Minute taking at meetings.
- SystmOne user skills.
- Effective time management (planning and organising).
- Ability to listen, empathize with people and provide person centered support in a non-judgmental way.
