About us

The South London Mental Health and Community Partnership (SLMHP) Complex Care Programme (CCP) is a collaboration between Oxleas Foundation Trust, South London and Maudsley NHS Foundation Mental Health Trusts and South West London and St Georges Mental Health Trust.

The programme aim is to transform the way decisions are made and how these are used to inform the design of care pathways to achieve improved outcomes for those with complex care mental health needs. A key focus is to ensure people are supported in the least restrictive setting, close to home &have a personalised outcomes-based support plan that builds on their skills and assets &community connections.

From 1st November 2020, the Programme received delegated authority as a Phase 1, to manage the 100% complex care health only funded placement budgets of the SE and SW London Integrated Care Boards (ICB) (excluding Lambeth). This includes managing complex care placements for c380 individuals at a cost of c£35m.

From April 2021, the Programme moved to a Phase 2 shadow year where it is working with borough pilot sites (a target of one in each Mental Health Trust has been set) in shadow form to support the boroughs to agree the health component of the joint funded placements (c16m) with Social Care. This has involved using the Programme's clinical knowledge and lessons learnt from Phase 1 to develop and invest in housing and support initiatives to support people in less restrictive settings and support value for money.
