Digital Transformation Lead

Job summary

Southend East PCN is looking for a Digital and Transformation Lead to help our member practices improve patient access, digital maturity, and the way services are delivered across the network. This is a permanent role funded through the NHS Additional Roles Reimbursement Scheme (ARRS), delivered in line with the requirements set out in the Network Contract DES.

While the role is advertised from our Norton Place base, the postholder will work flexibly across all Southend East PCN member practices, which are all within a close geographical area of one another. This isn't a single-site role you'll be working across the network to support collaborative improvement rather than being tied to one location.

Main duties of the job

    Hours: Full-time or part-time considered; exact hours to be agreed with the successful candidate

    Location: Flexible, PCN-wide role. Advertised from our Norton Place base, but the postholder will work across all Southend East PCN member practices, which are within a close geographical area of one another, as well as community settings and, where service delivery requires, patient homes and care homes

    DBS Check: Enhanced DBS check with barred list check required, reflecting the requirement to work in patient homes and care homes

    Job Summary

    The Digital and Transformation Lead supports increased access to care for patients across Southend East PCN by supporting the adoption of new technology and other initiatives that improve the care offer, and by enabling PCN staff to work more effectively to support the sustainability of general practice services.

    This is an ARRS role. The postholder's duties are drawn directly from the minimum role requirements for the Digital and Transformation Lead role set out in Annex B.17 of the Network Contract DES Specification 2026/27.

    Key Duties and Responsibilities

    Technology Adoption and Patient Access

    • Improve adoption of new technology to enhance patient access and experience and increase PCN productivity
    • Support the effective adoption of national and local digital and operational initiatives, including integrated working at neighbourhood and place level, to improve access to services for patients

    Practice Collaboration

    • Build relationships and facilitate collaboration between member practices to support the delivery of care to each other's patients, for example through enhanced access or other hub-type working arrangements

    Digital Maturity

    • Review and improve the PCN's digital maturity
    • Work with the Integrated Care System (ICS) and other partners to ensure that digital and operational PCN transformation is embedded in, and aligned with, ICS and national strategies

    Data and Insight

    • Use data, and improve data quality, to understand demand, capacity, and activity, driving improvements in patient experience of access, operational efficiency, and staff experience at work
    • Use data to support population health management and improvements in care quality and PCN operational efficiency
    • Understand the type and intensity of support needs across the PCN and coordinate this support, including through organisational development (OD) programmes
    • Present data insights to support service improvement and reduction in unwarranted variation

    Neighbourhood and Integrated Working

    • Work within Integrated Neighbourhood Teams (INTs), collaborating with general practice, community services, social care, and voluntary organisations
    • Support delivery of place-based, population health-focused care, and contribute to reducing health inequalities and improving access
    • Participate in multi-disciplinary team (MDT) meetings and neighbourhood forums as required

    Stakeholder Engagement and Communication

    • Build strong working relationships across PCN member practices and system partners
    • Engage staff in transformation and technology adoption programmes, and support clear communication of service changes to staff and patients

    Governance and Quality

    • Ensure transformation and digital activities align with the PCN's governance requirements
    • Support risk management and quality assurance processes relevant to transformation and digital projects
    • Monitor and report on performance against relevant PCN and DES objectives, maintaining accurate documentation

    Working Conditions

    • Cross-site and neighbourhood-based working across the PCN footprint
    • Travel across GP practices, community settings, patient homes, and care homes as service delivery requires
    • A combination of office-based, remote, and on-site working

    • The postholder must maintain confidentiality at all times in relation to staff, patient, and PCN business information, in accordance with data protection legislation and the PCN's data sharing and data processing arrangements
    • The postholder is required to work in accordance with the PCN's equality, diversity, and health and safety policies
    • This job description reflects the minimum role requirements for the Digital and Transformation Lead role under Annex B.17 of the Network Contract DES Specification. It is not exhaustive and may be reviewed and amended from time to time in consultation with the postholder, in line with any future changes to the DES

    Person Specification

    Experience

    Essential
    • Experience in operational management, transformation or programme delivery
    • Strong understanding of primary care/PCN systems
    • Experience of service improvement and change management
    • Excellent communication and stakeholder engagement skills
    • Ability to analyse and interpret data
    Desirable
    • Experience in digital transformation
    • Knowledge of the Network Contract DES and NHS Long Term Plan
