The Community Neurological Rehabilitation Team (CNRT B+H) is looking for an energetic, motivated, and highly organised Caseload Co-ordinator to drive the smooth running of our busy, specialist service in Brighton & Hove.

Join a dynamic Multi-Disciplinary Team delivering expert neurological rehabilitation to people recovering from stroke, brain injury, spinal injury, brain tumours, and long-term conditions such as multiple sclerosis and Parkinson's. Your actions will directly shape patient journeys and support individuals at crucial moments in their recovery.

Main duties of the job

What you'll do

In this fast-paced and rewarding role, you will:

  • Process incoming referrals quickly and accurately

  • Check that referrals meet service criteria and contain essential information

  • Conduct telephone triage and assessments when needed

  • Allocate referrals to the right professional and book their assessment without delay

  • Manage messages and enquiries, ensuring they reach the correct team member

  • Represent the service as the first point of contact with clarity and confidence

Every day, you'll coordinate, communicate, and prioritise referrals and messages to keep the service running smoothly.

Other duties include being the Sentinel Stroke Audit Programme (SSNAP) lead for data entry to a national audit platform at regular deadlines throughout the year.

You will have the opportunity to supervise a band 3 administrator.

Job description

Job responsibilities

aTo be the point of contact for CNRT B+H for referrers, professionals and those on the team caseload, dealing proactively with enquiries and escalating any concerns where necessary.

bTo process stroke referrals within the stroke pathway time frames and other referrals in a timely manner, to ensure a prompt response can be actioned by the team.

cTo be proactive in ensuring all of the relevant information is available to the professionals to assist them with their decision making and prioritisation.

Where appropriate, to undertake telephone triage reviews for new referrals to gather information and help determine the appropriate professional and intervention required.

To provide a person-centred approach to ensure all clients and carers views and opinions are considered and all people who make an enquiry are given respect, dignity and understanding.

To use IT systems for example SystmOne for inputting referrals, contacts and to track progress of current cases.

To collate and input client related (or other) data to produce relevant data for statistical analysis and reports using IT packages including Excel and Access.

To undertake surveys or audits as required by the team and to be one of the named SSNAP (National Stroke Survey) audit links.

Person Specification

Qualifications

Essential
  • GCSE passes at Grade C or above or equivalent experience
  • Knowledge of medical terminology
Desirable
  • IT/ Administration Qualification e.g. ECDL

Experience

Essential
  • Significant experience of working in a health or social care setting.
  • Previous administration experience
Desirable
  • Working with multi-disciplinary teams
  • Experience of working in a therapy setting as B3

skills and knowledge

Essential
  • Knowledge of social care & health systems, processes and pathways
  • Knowledge and use of recognised IT packages e.g. Microsoft Office, Excel and Access
  • Ability to meet conflicting deadlines, prioritise effectively and manage own workload
  • Ability to produce letters and reports
  • Proven interpersonal skills and appropriate empathetic and tactful manner
Desirable
  • Effective in using multiple database computer systems e.g. SystmOne
  • Customer care/reception training
  • Self-awareness of own strength and limitations
