Job summary
Bradford City 5 Primary Care Network is looking for an experienced and motivated PCN Data Quality Lead to help us improve the quality consistency and use of data across our eight GP practices
This is an exciting opportunity to take a leading role in strengthening how our PCN uses data to support patient care service delivery and improvement
Working closely with practice teams and the PCN management team you will help improve SystmOne data quality coding searches and reporting You will support practices to identify and resolve data quality issues improve appointment and activity data and develop reliable searches and reports that help us understand our populations and target services effectively
The role will be particularly important as primary care develops more proactive and neighbourhood based models of care Accurate data is essential to identifying patients who need support monitoring outcomes and demonstrating the impact of our services
We are looking for someone who combines strong SystmOne and primary care data skills with the ability to work collaboratively with practices explain data clearly and turn findings into practical improvements

Main duties of the job
The PCN Data Quality Lead will work across our eight GP practices to improve the accuracy consistency and usefulness of primary care data
A key part of the role will be working with practices to identify data quality issues and turn these into practical improvements You will develop and maintain SystmOne searches reports templates and coding processes and support teams to use these consistently
You will review appointment and activity data including GPAD and help practices identify issues with appointment book configuration slot mapping and recording that may affect the quality of reported data
The role will support the PCN to identify patient cohorts for proactive care long term conditions frailty dementia health inequalities and other agreed priorities ensuring that searches are accurate reproducible and useful to practices
You will produce clear reports and dashboards to help the PCN understand performance activity and population needs and support contractual and quality improvement work
You will work directly with practice managers clinicians administrative teams and the PCN management team providing guidance training and practical support to improve data quality
The postholder will also help establish consistent data quality standards and processes across the PCN and monitor whether improvements are being maintained.

Job purpose
The PCN Data Quality Lead will provide specialist hands-on data quality support across Bradford City 5 Primary Care Network and its eight member practices
The postholder will lead improvements in the accuracy consistency completeness and effective use of primary care data with a particular focus on SystmOne coding searches reporting appointment data and patient identification
The role will work closely with practice managers clinicians administrative teams and the PCN management team to identify data quality issues develop practical solutions and ensure improvements are embedded and maintained
A key purpose of the role is to ensure that the PCN has reliable data to support patient care contractual delivery population health management proactive care health inequalities service planning and developing Neighbourhood working
Data quality and coding
Undertake regular reviews of data quality across PCN practices and identify areas requiring improvement
Work with practices to improve the accuracy consistency and completeness of clinical and administrative coding
Identify variation in coding and recording between practices and support the development of consistent PCN approaches where appropriate
Investigate data quality issues and work with practice teams to understand the cause and implement practical solutions
Develop processes to monitor whether agreed data quality improvements are being maintained
Support practices to improve the quality of information used for patient identification reporting and service delivery
SystmOne searches and reporting
Develop maintain test and document SystmOne searches and reports for agreed PCN and practice priorities
Ensure searches are accurate reproducible and appropriately validated before they are used for operational or reporting purposes
Support practices to use agreed searches reports templates and coding processes consistently
Develop reliable patient searches to support proactive care long-term conditions frailty dementia health inequalities and other agreed PCN priorities
Review existing searches and reports and update or replace them where required
Maintain appropriate documentation so that key searches reports and data processes can be understood and reproduced
GPAD and appointment data
Support practices to improve the quality and accuracy of GP appointment data
Review appointment book configuration slot types mapping and recording processes that may affect GPAD and other reported appointment activity
Work with practice teams to identify incorrect or inconsistent appointment mapping and support corrective action
Monitor appointment data following changes to determine whether improvements have been achieved and sustained
Provide practical guidance to practice teams on accurate and consistent appointment recording
Performance and contractual support
Support the PCN management team and practices with accurate data for PCN DES IIF QOF and other agreed contractual or quality improvement requirements
Develop searches and reports to identify eligible patient cohorts and monitor progress against agreed priorities
Support validation of reported activity where data quality concerns are identified
Highlight data gaps inconsistencies and risks that may affect performance reporting or patient identification
Provide clear information to support planning decision making and improvement work across the PCN
Population health and proactive care
Use primary care data to support identification of patients who may benefit from proactive or targeted support
Support the development of reliable patient cohorts for agreed PCN programmes
Work with PCN teams and practices to improve the quality of data used for population health management and health inequalities work
Support emerging Neighbourhood working by helping ensure that relevant primary care data is accurate consistent and suitable for planning and service delivery
Help the PCN understand variation between practices and identify opportunities for targeted improvement
Reporting and insight
Produce clear regular reports and dashboards for the PCN management team and member practices
Present complex data in a clear and accessible way for clinical and non-clinical colleagues
Identify trends variation gaps and areas requiring further investigation
Provide appropriate interpretation and context alongside reports rather than presenting data without explanation
Work with practices to translate findings into practical actions and monitor the impact of agreed changes
Practice support and training
Build effective working relationships with practice managers clinicians administrative teams and other PCN staff
Provide practical support to practices to resolve identified data quality issues
Deliver appropriate guidance coaching and training on coding searches reporting appointment mapping and agreed data quality processes
Help practice teams understand how the quality of data entered into clinical systems affects patient care reporting performance and service planning
Encourage shared learning and consistent approaches across PCN practices
Work collaboratively and constructively recognising that practices may have different systems processes and levels of data quality maturity
Governance and information handling
Maintain confidentiality and comply with relevant information governance data protection and PCN and practice policies
Access patient information only where necessary and authorised for the responsibilities of the role
Ensure reports containing patient identifiable information are handled securely and only shared with authorised individuals
Use aggregated or anonymised information where appropriate
Maintain clear documentation of important searches reports definitions and data quality processes
Escalate significant data quality information governance or patient safety concerns through appropriate PCN or practice routes
Quality improvement
Take a structured improvement approach to identified data quality problems
Establish appropriate baseline information before significant improvement work where possible
Agree actions responsibilities and timescales with relevant teams
Measure the impact of changes and provide evidence of improvement
Identify recurring issues and develop sustainable solutions rather than relying solely on one-off data cleansing
Share effective approaches and learning across the PCN
Prioritise work according to patient safety contractual requirements operational impact and agreed PCN priorities
Working relationships
The postholder will work closely with the PCN Clinical Director PCN Managers practice managers GPs nurses administrative teams and the wider PCN multidisciplinary workforce
The role may also involve working with relevant external partners where required for agreed PCN data and reporting priorities
The postholder will need to build positive relationships across all eight practices and be able to provide constructive challenge where data quality or recording processes require improvement
Management and accountability
The postholder will be employed by Bradford City 5 PCN Limited and will work across the eight member practices
The postholder will work under the direction of the PCN management team and within agreed PCN governance arrangements
The role requires the ability to manage an agreed workload independently while maintaining regular communication with the PCN management team and practice teams
The postholder will provide regular updates on progress risks issues and measurable improvements
Training and development
Maintain and develop knowledge of SystmOne primary care data quality reporting and relevant contractual requirements
Complete required induction mandatory training and information governance training
Participate in supervision appraisal and continuing professional development
Attend relevant PCN practice and team meetings where required
Keep up to date with changes that affect primary care data recording reporting and patient identification and support practices to understand the implications

Qualifications
- Relevant degree level qualification or equivalent demonstrable experience in primary care data information management business intelligence health informatics or a related field.
- Evidence of relevant continuing professional development.
- Relevant training or qualification in data analysis health informatics quality improvement or primary care systems
- Training or recognised experience in SystmOne reporting searches or data management.
Experience
- Experience of working with primary care data and clinical information systems
- Strong practical experience of SystmOne including searches reporting coding and data quality work
- Experience of identifying investigating and resolving data quality issues
- Experience of developing and validating searches reports and patient cohorts
- Experience of analysing and presenting data to support service improvement performance monitoring or decision making
- Experience of working with a range of clinical and non-clinical colleagues to implement improvements
