Job summary
We are looking for a part time Long Term Conditions Administrator to work within our busy, friendly GP Practice.
This is a part time role for 19 hours a week. The work pattern will be:
- Tuesday 8.00 to 16.00
- Thursday 8.00 to 16.00
- Friday 8.00 to 12.00
The successful candidate will ideally have previous experience working in a GP practice, particularly within an administration, QOF or pharmacy team. They will be confident working with data and using Microsoft Excel, with excellent attention to detail and strong IT skills. The role requires someone who can work independently while contributing effectively as a team.

Main duties of the job
The post holder will work closely with the QOF team to support the delivery of the NHS Quality and Outcomes Framework (QOF) by coordinating and managing patient recall systems.
The role involves monitoring and co-ordinating recall programmes for patients requiring long-term condition reviews including heart failure, lipid disorders, primary and secondary prevention clinics, and DOAC monitoring. Responsibilities include contacting patients, arranging blood tests, booking appointments with the appropriate clinicians, and maintaining accurate patient records to ensure reviews and ongoing monitoring are completed in line with clinical and contractual requirements.
Full training will be provided.

- Coordinate patient recall systems to support achievement of NHS Quality and Outcomes Framework (QOF) targets
- Monitor and manage recall programmes for patients with long-term conditions, including heart failure
- Oversee Primary and Secondary prevention recall lists, including monitoring CHOL003 and CHOL004
- Identify patients due for reviews, arrange appropriate blood tests, and book appointments into relevant clinics
- Coordinate heart failure review appointments with Clinical Pharmacists for patients within HF003, HF006 and HF007 cohorts
- Monitor lipid disorder recall programmes, ensuring patients receive timely follow-up-blood tests for statin initiation and ongoing lipid management, within 2-3 month review periods.
- Manage Direct Oral Anticoagulant (DOAC) monitoring recalls, including arranging blood tests, weight checks and review appointments.
- Ensure accurate maintenance of patient recall registers and clinical records.
- Liaise with clinical and administrative teams to support effective delivery of chronic disease management services.

Person Specification
Qualifications
- Good standard of general education including English and Maths GCSE
- NVQ Level 3 in Healthcare Administration, Business Administration or equivalent qualification
Experience
- Previous experience working in a GP Practice, Primary Care Network or healthcare environment.
- Experience of using clinical systems including EMIS or equivalent.
- Experience of using Excel.
- Experience of handling confidential patient information.
- Experience of managing patient recall systems
- Experience supporting QOF achievement.
- Experience of working with Clinical Pharmacists and multidisciplinary clinical teams.
Knowledge and Skills
- Strong organisational and administrative skills with the ability to manage multiple recall programmes simultaneously.
- Excellent communication skills with the ability to interact professionally and sensitively with patients and healthcare professionals.
- Excellent IT skills, including Microsoft Office and clinical systems
