Job summary

You will be working for all of the surgeries in the Weymouth and Portland Primary Care Network (PCN).This is an exciting and developing role.

The role involves working closely with the PCN and the wider multidisciplinary team. The role will be pivotal in helping to deliver the best possible outcomes for patients and will maximise the potential presented by innovations in digital healthcare technology, and the associated new ways of working.

You will work along side the team of Care Coordinators who support member practices in coordinating key activity including access to services, advice, and information, and ensuring health and care planning is proactive, supportive, holistic and patient-centred. In particular, this role will help address the ever-increasing demand on health care services and the need to better support individuals within their own communities. To achieve this there is wide recognition of the need to embrace new ways of working and digital healthcare technologies to ensure all patients receive the right care, in the right place, at the right time.

You will need outstanding people skills and excellent IT skills with a desire to learn in this ever-changing environment

Main duties of the job

The role of the Digital and Data Coordinator will be to support patients and healthcare professionals to maximise the potential presented by new digital healthcare technology and the associated new ways of working, these include:

  • Healthcare Apps
  • Wearables and remote monitoring technology
  • Data and intelligence e.g. Population Health data, Dorset Care Record

Digital coordination is a mechanism for delivering 21st Century care and connecting people with both medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.

Job responsibilities

Key Responsibilities

  • Be an ambassador for Digital with patients and colleagues in equal measure
  • Act as the first point of contact for digital queries
  • Be an ambassador for population health management with colleagues and be able to use understanding of this to support in the reduction of health inequalities
  • Work with practice teams, primary care professionals and wider community teams to identify patients who would benefit from access to digital initiatives
  • Maximise the digital literacy of your Primary Care Networks (PCN) population and actively move them to a Digital First approach
  • Run regular searches and / or Population Health Intelligence and Risk Stratification tools (predominantly on SystmOne and via DiiS) to identify target groups for interventions
  • Signpost team members, patients and carers to relevant digital services or apps as appropriate
  • Support patients who need assistance to onboard onto digital apps to support their independence, promote healthcare and well-being
  • Support patients and carers to utilise decision aids when appropriate
  • Assist people to access online self-management education courses, peer support or interventions that support them to take more control of their health and wellbeing
  • Use healthcare technologies to optimise service delivery, access, and continuity of care for patients and carers
  • Support the social media campaigns across the network and develop promotional material to increase uptake of existing and new digital healthcare technologies
  • Lead on streamlining online messaging to patients across the PCN , linking websites and social media campaigns where possible
  • Maintain and develop the PCN website
  • Support with the roll out of BP@home
  • Take the lead in the deployment of new technologies and education of patients
  • Attend local initiatives e.g monthly digital care coordinator community of practice and Digital access to services at home Teams meetings

Person Specification

Experience

Essential
  • Data analysis and interpretation
  • Experience of working with patients or the public in a customer care environment.
Desirable
  • Primary Care experience
  • Working in a healthcare setting as part of a team

Qualifications

  • Good general secondary education including Maths and English
Desirable
  • Presentation skills
  • Change management

Knowledge and skills

Essential
  • Good working knowledge of primary care and the challenges facing patient care in the NHS.
  • A good range of IT knowledge including Word, Excel, email and databases,
  • IT GP clinical system, SystmOne
  • Knowledge of the Dorset Primary Care Digital Strategy
  • Understanding and adherence to strict confidentiality
  • Excellent communication and interpersonal skills, both written and verbal
  • Good teamworking skills
  • Able to analyse & extract relevant information
