Hepatology Testing and Treatment Coordinator - Band 4

Job summary

TheHepatitis C Team has an exciting opportunity for a Hepatology Testing and treatment Coordinator to join our team.

Reporting to Hepatitis C Lead Nurse, the post holder will be responsible for identification and engagement of individuals and/or groups who would benefit from HCV testing and/or curative treatment for the Hepatitis C Operational Delivery Network (ODN) across Lincolnshire.

Projects include HCV testing and treatment projects across community, outreach and health and justice settings, primary care and drug and alcohol treatment services.

The post holder will also coordinate pathways of care that are individual and person centred. Provide clinical interventions such as fibro scanning and phlebotomy in the hospital and the community. Working across Lincolnshire to motivate and encourage staff within outreach and community settings (e.g. drug and alcohol services etc.) to carry out HCV testing.

The ideal candidate will have a primary or secondary care background and/or have experience of working within drug and alcohol treatment services.

Main duties of the job

To undertake case finding initiatives to identify patients with hepatitis C, working with GP practices, laboratory databases and other organisations.

To liaise with patients to encourage them to enter care and to manage their pathway, ensuring a convenient date, time and location time is agreed.

To facilitate the prioritisation of HCV positive individuals by supporting the coordination of assessments at outreach clinics and linkage into care.

To support the Hepatitis C and Department of Hepatology by taking blood and Fibro scanning patients at outpatient and outreach clinics within Lincolnshire.

To monitor the performance of HCV testing by organisations in the community e.g. drug and alcohol and sexual health clinics.

To deliver capillary blood test training within HCV outreach clinics.

To maintain a record of the HCV test training within the ODN outreach clinics.

To be an effective part of the multi-disciplinary team contributing to the departmental objectives and the Hepatitis CQUIN and milestones.

To maintain supportive and effective relationships with partners including community drug and alcohol services, community pharmacists, GP practices, and homeless centres.

To use patient notes, existing databases, and the hospital systems to confirm patient data is up-to-date and accurate.

Person Specification

Qualifications

Essential
  • Experience in a care environment
  • GCSE's/NVQ or equivalent
Desirable
  • Vocationally based education
  • Training in motivational interviewing

Previous experience

Essential
  • Experience of motivating and engaging staff.
  • Minimum of 12 months NHS experience or other public/third sector organisations.
  • Knowledge and understanding of hepatitis C infection.
  • Experience of health promotion and harm reduction across diverse communities.
  • Experience of working with a diverse range of people.
  • Experience of working innovatively both as an individual and in a team.
  • Competent in Microsoft Office; including Word and Excel Databases
Desirable
  • Experience of motivating and engaging staff in external organisations
  • Experience of working with service users with complex needs.
  • Experience of developing training materials.
  • Knowledge of health inequalities.
  • Ability to extract and analyse data to deliver outcomes.

  • Experience of phlebotomy such as venepuncture, capillary blood testing, dry blood spot testing etc.
  • Ability to acquire in-depth knowledge of Hepatitis C Operational Delivery Networks (ODN), protocols and communicate this to lay persons
  • Understanding of patient pathway problems.
  • Ability to analyse simple data, all of which require attention to detail and accuracy.
  • Willingness and ability to learn new clinical skills such as Fibro scanning.
  • High level of planning and organisational skills with an adaptable approach to meet challenging circumstances, maintaining a positive attitude and identifying solutions.
  • Demonstrate effective written and verbal communication skills.
  • Ability to evidence/demonstrate key values and behaviours in line with the Trust framework: oPatient Centred oSafety oCompassion oRespect and oExcellence

Specific requirements

Essential
  • Able to develop close links with the Viral Hepatitis and Hepatology teams in Lincolnshire, the Operational Delivery Network, Substance Misuse Services, Prison and GP surgeries.
  • Able work as an individual and in a team.
  • Ability and willingness to travel across Lincolnshire and work within a variety of community settings.
