Job summary

Assist in the delivery of a high standard of patient care, observing and reporting specific changes anddocumenting care given. Assist senior staff with coordinating department flow, liaising with bedmanagement team, updating FirstNet/e-Care, assisting with patient transfers and other duties as required.Delivery of careo Ensure own actions are consistent with clinical governance systems and practice in accordance withagreed standards of careo Adhere to the basic principles of infection control demonstrating effective hand hygiene and awareness ofpotential cross infection.o Accompany patients to other wards/departments as required.o Carry out clinical interventions identified as part of the role and for which competency has been assessed, including recording of ECGs, phlebotomy, cannulation and blood glucose testing, documentingand reporting the results o Demonstrate basic cleaning and decontamination of communal equipment between patient use and / orstorage.o Carry out minor treatments for patients, for example: wound care and simple dressings.o Accurately use IT systems to record observations, treatments and all aspects of care o Follow trust policies in accordance with the Trust objectives

Job responsibilities

Communication and team working Communicate effectively and sensitively with patients, relatives and colleagues, referring to senior staff asnecessary, recognising needs for alternative methods of communication. Work as part of the team to facilitate good communication between all departments. When appropriate and under the direction of ward staff, take action to expedite investigations and treatment.e.g. ensure patients are logistically moved to diagnostic areas within a timely fashion, or liaise with EIT toensure of a patients timely review. Liaise with the medical team and pharmacy staff to ensure discharge medication is prescribed and dispensedpromptly. Answer the telephone and relay messages accurately, courteously and competently. Welcome patients and visitors in a professional, friendly and helpful manner. Work effectively as a member of the department team, offering support to colleagues and requestingassistance as necessary. Record care given accurately, legibly, completely and concisely in the patients notes Contribute to the induction, training and development of non-registered staff within the area. Privacy, dignity, equality Act to promote patients equality, diversity and rights at all times. Ensure provision of privacy and dignity in all activities. Ensure all care is individualised and delivered with the informed consent of the patient. Maintain confidentiality of patient information at all times. Administrative Ensure up to date records for all newly assessed patients are available for all staff. Ensure investigations are carried out and results are available when required. e.g. blood sampling, ensuringpatients attend X-ray at the given allotted time, microbiology / laboratory samples are sent in the appropriatemanner, as not to delay results. Support unit staff in the identification of delayed transfers of care, ensuring that accurate records regardingthe reasons for the delay are recorded. Ensure self-discharge patients, or absconding patients matters, aredealt with in accordance to Trust policy. Ensure appropriate management and security of patients property. Enter data into computerised systems as required (e.g. FirstNet or Powerchart). Undertake errands off the ward as required e.g. to pharmacy, x-ray, patient transfers to their residence. Report repairs/maintenance issues appropriately and ensure unit upkeep is maintained. Demonstrate insight into own development needs and access development through an agreed PersonalDevelopment Plan. Attend mandatory training, and other in-service training as necessary. Health and safety Use and encourage others to be familiar with the personal security systems within the workplace accordingto practice guidelines Identify the risks involved in work activities and undertake them in a way that manages the risks Know the health and safety policies and procedures within the workplace, including fire procedures,maintaining documentation, monitoring and maintaining of equipment and furniture within your area ofresponsibility

Person Specification

Education and qualifications

Essential
  • Good general education Including Maths and English at GCSE grade C/Level 4 or above (or equivalent)
  • NVQ level 3 or working towards.
Desirable
  • National Care Certificate holder
  • Functional skills level 1 Maths and English or GCSE equivalent

Experience and knowledge

Essential
  • Recent relevant experience of working in the acute healthcare environment
  • Experience of working/communicating with the general public
  • Ability to communicate with a range of different people, including service users, carers and members of the multi-disciplinary team
  • Understanding of team working
