Job summary

The objective of the Digital Transformation & PCN Manager is to provide a central source of leadership and management to aid service transformation for our network population.

The ideal candidate will need to be proactive, have a passion for Primary Care, and have a proven track record in delivering Primary Care improvements.

The role will involve line management of the PCN team, managerial responsibilities, and leadership for the PCN and systemwide networking.

You will also play a key role in ensuring delivery of national PCN contractual requirements (e.g. DES, IIF), working alongside clinical and practice leaders to identify service improvements, implement digital innovations, and lead workforce development.

The remit of this role is broad and will require attention to detail in delivering high-quality day-to-day operations

MAIN RESPONSIBILITIES

STRATEGIC PLANNING

To lead and develop a strategic plan that includes a robust digital infrastructure

To clarify objectives and priorities for network delivery in line with operational capacity, financial parameters, and contract requirements

Promote the engagement of all PCN practices and act as an ambassador for the network.

To proactively build relationships with system leaders to help influence proactive and new models of care

OPERATIONS

To provide oversight and accountability to the delivery of PCN contracts and specifications e.g. DES, IIF, Enhanced Services

Coordinate training and development of PCN staff

Identify and record risks and issues and develop contingency plans with Network Leads

Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented

To drive issues through to resolution, acting as a point of expertise

To implement network-based projects

Organise and facilitate PCN meetings, including Board and ARRS supervision meetings, ensuring agendas, minutes, and actions are completed and followed up.

FINANCE AND GOVERNANCE

To support the collation of financial information

To monitor and check the accuracy of the submissions for the DES and contractual requirements

Compiling and presenting financial reports

To ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness.

Monitor IIF income achievement, track financial forecasting across the PCN, and ensure PCN funding is deployed equitably and transparently.

DIGITAL INFRASTRUCTURE

To ensure the PCN Maximises its business intelligence tools

Develops a continuous improvement approach to digital accessibility for our patients and workforce

Creates data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff, and patient experience

Support practice digital leads and work closely with IT providers, digital enablers and ICB transformation teams to ensure alignment with the wider digital strategy.

RELATIONSHIP MANAGEMENT:

To foster a culture of collaboration and promote learning to drive continuous improvement

To work as a flexible member of the PCN, providing support to other team members when necessary

Develop and deliver a communication plan using a wide variety of channels

To take an active role in the development and embedding of the PCN's culture, values, and reputation

To represent the network at external meetings

To represent the network at external meetings, including ICB locality forums, PCN Manager groups, and partner organisations.

Build relationships with local community services, secondary care, pharmacies, and voluntary organisations to support integrated service delivery.

HR AND WORKFORCE PLANNING

Support recruitment, induction, and training of new PCN staff

Support recruitment which is aligned to a workforce plan

To facilitate progression for the PCN staff to promote retention

Provide day-to-day line management to ARRS staff and facilitate performance reviews, supervision sessions, and staff wellbeing support.

Maintain up-to-date records relating to PCN staff supervision, DBS, training compliance, and sickness monitoring.

Person Specification

Qualifications

Desirable
  • Education to degree level or equivalent professional experience
  • Quality improvement training
  • Project management qualification (e.g. PRINCE2, Agile, Lean, etc).

Experience

Essential
  • Experienced in delivering multiple programmes of IT and general projects through the entire project lifecycle, with examples
  • 2. Experience in delivering projects within or across multiples sites and departments
  • Good understanding of the Digital agenda within the NHS and Social Care or Care Sector
  • Experience of managing change within complex environments
  • People management experience, including performance and HR processes.
  • Experience of delivering targets and working to deadlines, providing feedback and updates.
Desirable
  • NHS or primary care-based project management experience.
